O córtex pré-frontal ventrolateral esquerdo (CPFVLe) é apontado como fundamental ao sucesso do processamento da reavaliação cognitiva, a mais estudada estratégia de regulação emocional, porém são escassas as evidências neurobiológicas sobre sua participação. Objetivo: Investigar a participação do CPFVLe na reavaliação cognitiva, através da Estimulação Transcraniana por Corrente Contínua (ETCC) online em estudantes. Métodos: Ensaio clínico randomizado, sham-controlado, duplo-cego, que comparou os dados de 36 voluntários destros divididos em grupos: ETCC anódica (n=11), catódica (n=14), sham (n=11). Aplicou-se uma corrente [1mA; 20 min (ETCC real) ou 30 seg (sham)] entre o eletrodo “ativo” (22,5cm²; CPFVLe) e o de “retorno” (60cm²; região supraorbital direita). A ETCC foi realizada durante tarefa cognitiva, que consistia em regular as emoções pela tática de reinterpretação diante de imagens, sob as condições: aumentar, diminuir, manter emoções negativas e manter emoções neutras. Utilizaram-se quatro séries de 15 figuras do International Affective Picture System – uma neutra, para diminuir a habituação emocional; e três negativas, pareadas por valência e alerta (arousal). Os participantes avaliaram de 1-7 a intensidade do arousal após aplicar a instrução recebida a cada figura. Resultados: ANOVA com comparações múltiplas de Tukey revelou diferença significativa de arousal apenas entre os grupos ETCC anódica (4,10 ± 0,77) e sham (3,06 ± 0,72) para a condição diminuir (ρ=0,004). ANOVA de medidas repetidas com comparações múltiplas de Bonferroni demonstrou diferenças significativas de arousal entre todas as condições de regulação, indicando sucesso na implementação da regulação emocional em todos os grupos (ρ<0,001), exceto no grupo ETCC anódica entre as condições diminuir e manter. Conclusão: A ETCC anódica online sobre o CPFVLe reduziu a capacidade de diminuir emoções negativas através da reinterpretação em estudantes. É necessário reproduzir estes achados em maiores amostras para confirmá-los.Introduction: Dysfunctions in emotion regulation play a core role across psychiatric disorders. Ventrolateral prefrontal cortex (lVLPFC) has been implicated as a crucial area in successful cognitive reappraisal, the most studied strategy of emotion regulation. However, there is poor neurobiological evidence in literature about its participation in this process. Objective: To investigate the participation of lVLPFC in cognitive reappraisal by applying online transcranial Direct Current Stimulation (tDCS) over this area in students. Methods: Doubleblind, sham-controlled clinical trial was performed and data from 36 right-handed volunteers were analyzed. Participants were randomly allocated into groups: anodal (n=11), cathodal (n=14), sham (n=11) stimulation. tDCS [1mA; 20 min (real stimulation) or 30 sec (sham)] was applied through a pair of electrodes: over lVLPFC (“active”; 22,5cm²) and right supraorbital area (“return”; 60cm²). The task consisted in regulating emotions elicited by images under four conditions: upregulate, downregulate, maintain negative and maintain neutral emotions. During tDCS, volunteers were presented to four series of 15 images from the International Affective Picture System – one neutral, to attenuate emotional habituation; and three negative, matched according to valence and arousal. Subjects ranked from 1 to 7 the intensity of emotional arousal after reinterpreting each picture. Results: ANOVA with the Tukey Multiple Comparison test revealed differences of arousal only between anodal (4,10 ± 0,77) and sham (3,06 ± 0,72) groups for downregulate condition (ρ=0,004). Repeated measures ANOVA with Bonferroni correction demonstrated significant changes of the arousal intensity for all regulation conditions, indicating successful emotion regulation in all groups (ρ<0,001), except for the anodal group, which showed no significant differences between downregulate (4,10 ± 0,77) and maintain (4,58 ± 0,87) conditions. Conclusion: Anodal online tDCS over lVLPFC impaired the ability to downregulate negative emotion through reinterpretation in healthy individuals.
