Efetividade da Telenfermagem no processo adaptativo de pessoas com estomia sob a perspectiva do Modelo de Adaptação de Roy

Resumo: Stomy is the name given to any artificially created opening of the internal environment of the body structure to the external environment. The fact of becoming stomped causes changes related to the new excreta elimination condition, generating the need to adapt to this situation. The difficulty of access to care may result in the individual's reduced knowledge of care for the oedematous patient. In this study, we chose to study the effectiveness of an intervention based on the follow-up of the person with oedematous over the telephone, evaluated and systematically directed using the Roy's Adaptation Model as a nursing actions supervisor. The objective of this study was to analyze the effectiveness of the follow-up intervention by telenfermagem in the adaptive process of the person with the stoma. This is a longitudinal, randomized, controlled, clinical and unicex study conducted with two groups of people with up to one year of oesophageal filling, with a sample of 33 individuals. The control group received only conventional treatment and the intervention group obtained, besides conventional treatment, the follow-up via telephone contact. Descriptive and inferential statistics were used to analyze the data. The results showed that there was no statistically significant difference between the control and intervention groups, as well as their sociodemographic, clinical and adaptation variables, the sample characterization, the predominance of males aged less than or equal to 60 years old was identified, pardos, retired, living with a companion, with education degree up to elementary school, with income of up to 1 minimum wage, Catholics. According to the clinical and health aspects, the majority do not have diseases or injuries, did not undergo chemotherapy or radiotherapy, had intestinal cancer as the predominant diagnosis of stoma manufacture, have no stoma complications and peristomal skin, have a colostomy, uses the collector device is in one piece, the patient is performing the exchange of his bag, and feels adapted. Regarding the adaptive modes, we identified the absence of a statistically significant difference between the control and intervention groups regarding their adaptation in the pre-test. However in the post-test it was pointed out that the means and medians of the control group were lower than the measures of the group that received the intervention in a statistically significant way. In the intervention group, there were better scores of the adaptive levels in the post-test when compared to the pre-test. It is noticed the effectiveness of a telephone intervention in the levels of adaptation of the person with stoma, emphasizing the importance of a complementary and regular accompaniment in the promotion of health actions that influence in the self-esteem, self-care and adaptation of this public. Thus, the postoperative follow-up intervention in the light of Roy's adaptive model, via telephone contact, showed an improvement in the adaptation levels of people with stomies compared to the control group.

A dificuldade de acesso aos atendimentos pode resultar no reduzido conhecimento do indivíduo sobre os cuidados com a estomia, diante disso optou-se por estudar nessa pesquisa a efetividade de uma intervenção baseada no acompanhamento da pessoa com estomia via telefone, avaliada e direcionada sistematicamente utilizando o Modelo de Adaptação de Roy como um orientador das ações de enfermagem. Objetivou-se analisar a efetividade da intervenção de acompanhamento via telenfermagem no processo adaptativo da pessoa com estomia. Trata-se de um estudo longitudinal, do tipo ensaio clínico, controlado, randomizado e unicego realizado com dois grupos de pessoas com até um ano de estomia, totalizando a amostra de 33 indivíduos. O grupo controle recebeu apenas o tratamento convencional e o grupo intervenção obteve além do tratamento convencional, o acompanhamento via contato telefônico. Utilizou-se a estatística descritiva e inferencial para análise dos dados. Os resultados apontaram que não houve diferença estatisticamente significante entre os grupos controle e intervenção quanto suas variáveis sociodemográficas, clínicas e de adaptação, a caracterização da amostra, identificou-se a predominância de pessoas do sexo masculino, com idade menor ou igual a 60 anos, pardos, aposentado, vivendo com companheiro, com grau de instrução até ensino fundamental, com renda de até 1 salário mínimo, católicos. De acordo com os aspectos clínicos e de saúde a maioria não possui doenças ou agravos, não realizou quimioterapia ou radioterapia, teve câncer intestinal como diagnóstico predominante da confecção do estoma, não possui complicações de estoma e pele periestomal, possui colostomia, com caráter temporário, utiliza o dispositivo coletor é de uma peça, sendo o paciente que realiza a troca de sua bolsa, e se sente adaptado. No tocante aos modos adaptativos, identificou-se a ausência de diferença estatística significante entre os grupos controle e intervenção quanto sua adaptação no pré-teste. Entretanto no pós-teste foi apontado que as médias e medianas do grupo controle foram mais baixas do que as medidas do grupo que recebeu a intervenção de modo estatisticamente significante. Evidenciou-se no grupo intervenção melhores escores dos níveis adaptativos no pós-teste quando comparado ao pré-teste. Percebe-se a efetividade de uma intervenção telefônica nos níveis de adaptação da pessoa com estomia, ressaltando a importância de um acompanhamento complementar e regular na promoção de ações de saúde que influenciam na autoestima, autocuidado e adaptação desse público.
