Percepção dos profissionais do NASF-AB sobre o processo de trabalho, seus avanços e desafios

Resumo: The NASF-AB were created with the objective of strengthening, integrating and expanding the activities developed by the Health Familly , being a specialized support of performance guided by Matrix Support. In the work routine, there are some obstacles for the NASF-AB to be effective in the face of the health situations of the territories it supports. Thus, the objective of this study is to analyze the NASF-AB work process in a northeastern capital, based on the perception of its professionals. This is a case study with a qualitative approach, carried out with 11 professionals. For data collection, the semi-structured interview and empirical field observation were used. The data were processed through Content Analysis, categorizing the findings using the Thematic Analysis modality. All participants were publicly contested in the municipality, distributed in three NASF-AB teams, the majority being female (n = 9), with an average age of 56 years. The professional categories that were interviewed include Physiotherapists, Physical Education Professionals, Nutritionists, Psychologists and Social Workers. From the analysis of the empirical material, five thematic categories emerged: The NASF-AB work agent; labor relations and their implications for health practices; obstacles to the incorporation of NASF-AB's means of work; presentation of the work scenario of NASFAB professionals; and the paths taken, the future and uncertainties. According to the data obtained, there was no training in health that would instrumentalize professionals for SUS, the work with the supported teams develops in a distant way, with isolated actions, and the work relations with the management are marked by a lack of dialogue between peers and pressure for productivism. Regarding Matrix Support, professionals recognize their conceptual aspects, but there is still the reproduction of the logic of referral and a defense by activities of assistance nature. Work tools are also recognized, but underutilized in conducting cases. In the groups conducted by the NASF-AB, although they did not present a defined methodology, efforts were noticed to incorporate practices based on Popular Education in Health. The work scenario has some limitations, such as the precariousness of the Health Care Networks and lack of inputs and materials for the development of activities. Even so, it is evident that the provision of NASF-AB services has broadened the scope of the ESF's actions and the professionals note that it is not possible to discuss resolvability of the service without mentioning points that interfere with the work developed. Faced with the new PNAB, some professionals feel comfortable prioritizing individual care and others recognize the threat of losing the matrix function of the Centers. NASF-AB still faces many difficulties in establishing itself as decisive and essential for PHC in Brazil. It is necessary to review the guiding documents of the work so that its performance is enhanced, redirecting the practices for the defense of Matrix Support as guiding this work, in order to strengthen the permanence of the NASF-AB with the Family Health of Brazil.

No cotidiano de trabalho, existem alguns entraves para que o NASF-AB seja efetivo diante das situações de saúde dos territórios que apoiam. Dessa forma, o objetivo desse estudo é analisar o processo de trabalho do NASF-AB de uma capital nordestina, a partir da percepção de seus profissionais. Trata-se de um Estudo de Caso de abordagem qualitativa, realizado com 11 profissionais. Utilizou-se, para a coleta de dados, a Entrevista Semiestruturada e Observação de Campo. O tratamento dos dados se deu por meio da Análise de Conteúdo, categorizando os achados por meio da modalidade de Análise Temática. Todos os participantes eram concursados, estão distribuídos em três equipes NASF-AB, sendo a maioria do sexo feminino (n=9), com uma média de idade de 56 anos. As categorias profissionais que foram entrevistadas contemplam Fisioterapeutas, Profissionais de Educação Física, Nutricionistas, Psicólogas e Assistentes Sociais. Da análise do material empírico, emergiram cinco categorias temáticas: O agente de trabalho do NASF-AB; as relações de trabalho e suas implicações nas práticas de saúde; os entraves para a incorporação dos meios de trabalho do NASF-AB; apresentando o cenário de trabalho dos profissionais do NASF-AB; e os caminhos percorridos, o futuro e as incertezas. Segundo os dados obtidos, não houve uma formação em saúde que instrumentalizasse os profissionais para o SUS, o trabalho junto às equipes apoiadas se desenvolve de maneira distante, com ações isoladas, e as relações de trabalho com a gestão são marcadas por falta de diálogo e pressão por produtivismo. Referente ao Apoio Matricial, os profissionais reconhecem seus aspectos conceituais, mas ainda existe a reprodução da lógica do encaminhamento e uma defesa pelas atividades de cunho assistencial. As ferramentas de trabalho também são reconhecidas, mas subutilizadas nas conduções dos casos. Já nos grupos conduzidos pelo NASF-AB, embora não apresentassem metodologia definida, percebeu-se esforços para a incorporação de práticas pautadas na Educação Popular em Saúde. O cenário de trabalho tem algumas limitações, como a precariedade das Redes de Atenção à Saúde e falta de insumos e materiais para o desenvolvimento de atividades. Mesmo assim, fica evidente que a oferta dos serviços do NASF-AB ampliou o escopo das ações da ESF e os profissionais atentam que não é possível discutir resolutividade do serviço sem citar pontos que interferem no trabalho desenvolvido. Diante da nova PNAB, alguns profissionais se sentem confortáveis para priorizar os atendimentos individuais e outros reconhecem a ameaça de perder a função matricial dos Núcleos. O NASF-AB enfrenta, ainda, muitas dificuldades para se estabelecer como resolutivo e essencial para a APS no Brasil.
