Resumo: Introduction: Actinic cheilitis (AC) is an inflammatory condition of the lips caused by chronic exposure to ultraviolet radiation and it is considered a potentially malignant disorder by the World Health Organization (WHO). Objective: develop and test a clinical index for Actinic Cheilitis (ACCI). Methodology: Based on a literature review, a questionnaire composed by 35 clinical characteristics of the AC was elaborated, which were selected and associated with a clinical severity score by a group of 52 surgeon-dentists. After establishing exclusion criteria, 16 characteristics were selected to compose the index. Twenty patients were selected to compose our sample after application the ACCI and incisional biopsy. The biopsied specimens were classified according to the histopathological grading for oral epithelial dysplasia of the WHO (EL-NAGGAR et al., 2017) and the Binary System (KUJAN et al., 2006). The association between the clinical characteristics and the grade of histopathological severity of the lesion was analyzed using Fisher's Exact Test (p <0.05). The precision of the index was tested based on the percentage of sensitivity, specificity, positive predictive value (PPV), negative predictive value (NPV), accuracy and ROC curve to cut-off points 5, 7, 9, and 11. Results: There was a prevalence of males (80 (90%), whites (90%), with occupational sun exposure (75%). The most frequent clinical characteristics were loss of lip / skin demarcation (100%), dryness (85%), white spot (80%) and flaking (65%). There wasn`t statistically significant association between the clinical characteristics of AC and histopathological gradations analyzed. The cut-off point that presented the best performance for the ROC curve and the highest percentage for specificity, PPV and accuracy was the value 11. Conclusion: The cut-off point 11 obtained the best precision for the ACCI, and this index value was indicated for biopsy, since it presented smaller number of false-positive individuals according to the two histopathological grading analyzed.

Objetivo: Desenvolver e testar um Índice Clínico para QA (ICQA). Metodologia: Com base em uma revisão da literatura foi elaborado um questionário composto por 35 características clínicas da QA, as quais foram selecionadas e associadas a um escore de severidade clínica por um grupo de 52 cirurgiões-dentistas. Após o estabelecimento de critérios de exclusão e associação foram selecionadas 16 características para compor o índice. Vinte pacientes foram selecionados para compor nossa amostra após aplicação do ICQA e realização de biópsia incisional. Os espécimes biopsiados foram classificados de acordo com a gradação histopatológica para displasias epiteliais orais da OMS (EL-NAGGAR et al., 2017) e do Sistema Binário (KUJAN et al., 2006). A associação entre as características clínicas e o grau de severidade histopatológica da lesão foi analisada através do Teste Exato de Fisher (p < 0,05). A precisão do índice foi testada com base no percentual de sensibilidade, especificidade, valor preditivo positivo (VPP), valor preditivo negativo (VPN), acurácia e curva ROC. Resultados: Houve uma prevalência de indivíduos do sexo masculino (80%), com idade igual ou superior a 40 anos (90%), raça branca (90%), com exposição ocupacional ao sol (75%). As características clínicas mais frequentemente encontradas foram perda da demarcação lábio/pele (100%), ressecamento (85%), mancha branca (80%) e descamação (65%). Não houve associação estatisticamente significativa entre as características clínicas da QA com as gradações histopatológicas analisadas. O ponto de corte que apresentou melhor desempenho para curva ROC e maior percentual para especificidade, VPP e acurácia foi o valor 11.
