Subconjunto terminológico da Classificação Internacional para a Prática de Enfermagem (CIPE® ) para o cuidado à gestante durante o trabalho de parto

Resumo: The use of the nursing process contributes to the continuity of care and comprehensive care. Taking into account the reduced number of nursing diagnostic concepts specifically aimed at care during labor, in addition to the lack of a terminological subset of the International Classification for Nursing Practice (ICNP® ) on the subject and the importance of the nursing process, nursing and the systematization of care, this dissertation aimed to develop a terminological subset of the ICNP® for the care of pregnant women during labor. The research was carried out through a methodological study consisting of four stages and with the participation of specialist professionals who work in the care of pregnant women during labor, to evaluate and validate the terms found. The steps were: 1) identification of relevant terms; 2) cross-mapping of identified terms with ICNP® terms; 3) selection of precoordinated Diagnosis (ND), Results (NR) and Nursing Interventions (NI) statements; 4) structuring of the terminological subset. In the first stage, relevant terms about the theme were collected through integrative review and analysis with the IRaMuTeQ software, which underwent a validation process to certify their relevance for clinical practice; in the second, a cross-mapping of the validated terms with the terms contained in the ICNP® was carried out; in the third stage, the statements ND/NR and NI were selected; and, in the last stage, the terminological subset was built with the statements, subdivided according to Horta's Theory of Basic Human Needs. The integrative review resulted in 19 articles for analysis. After the exhaustive and complete reading of the 19 articles, and the analysis of the IRaMuTeQ software, 39 terms were found to compose the validation instrument. Cronbach's alpha reliability test was performed to assess the reliability level of the research instrument, totaling an alpha value of 0.957, resulting in an internal consistency considered excellent. Validation was carried out by 20 expert evaluators in the area and, after evaluating the instrument, 24 terms were validated. After crossing validated terms and precoordinated ICNP® concepts, 15 terms were associated with diagnostic statements, resulting in 21 Nursing Diagnosis statements and 167 Nursing Interventions associated with these ND, thus composing the Terminological Subset, divided according to Basic Human Needs and their domains, from Wanda Horta's Theory. The elaboration of the ICNP® Terminology Subset for the care of pregnant women during labor aims to contribute to nursing care through the construction and availability of a tool with standardized and unified language in order to support nursing practice based on evidence and favor the implementation of the nursing process, directing the actions of professionals in the area.

Levando em consideração a reduzida quantidade de conceitos diagnósticos de enfermagem voltados especificamente para o cuidado durante o trabalho de parto, somada à inexistência de um subconjunto terminológico da Classificação Internacional para a Prática de Enfermagem (CIPE® ) sobre o tema e a importância do processo de enfermagem e da sistematização da assistência, a presente dissertação teve como objetivo desenvolver um subconjunto terminológico da CIPE® para o cuidado à gestante durante o trabalho de parto. A pesquisa foi realizada por meio de um estudo metodológico constituído em quatro etapas e contou com a participação de profissionais especialistas que atuam no cuidado à gestante durante o trabalho de parto, para avaliar e validar os termos encontrados. As etapas foram: 1) identificação de termos relevantes; 2) mapeamento cruzado dos termos identificados com termos da CIPE® ; 3) seleção de enunciados Diagnósticos (DE), Resultados (RE) e Intervenções de Enfermagem (IE) pré-coordenados; 4) estruturação do subconjunto terminológico. Na primeira etapa, foram coletados termos relevantes acerca da temática através de revisão integrativa e análise com o software IRaMuTeQ, que passaram por processo de validação para certificar sua relevância para prática clínica; na segunda, foi realizado um mapeamento cruzado dos termos validados com os termos contidos na CIPE® ; na terceira etapa, foram selecionados os enunciados DE/RE e IE; e, na última etapa, foi construído o subconjunto terminológico com os enunciados, subdivididos de acordo com a Teoria das Necessidades Humanas Básicas de Horta. A revisão integrativa resultou em 19 artigos para análise. Após a leitura exaustiva e na íntegra dos 19 artigos, e a análise do software IRaMuTeQ, foram encontrados 39 termos para compor o instrumento de validação. Realizou-se o teste de confiabilidade alfa de Cronbach para avaliar o nível de confiabilidade do instrumento de pesquisa, totalizando o valor de alfa de 0,957, resultando em uma consistência interna considerada excelente. A validação foi realizada por 20 avaliadores expertises na área e, após a avaliação do instrumento, 24 termos foram validados. Após o cruzamento entre termos validados e conceitos pré-coordenados da CIPE® , 15 termos foram associados a enunciados diagnósticos, resultando em 21 enunciados Diagnósticos de Enfermagem e 167 Intervenções de Enfermagem associadas a estes DE, compondo assim o Subconjunto Terminológico, dividido de acordo com as Necessidades Humanas Básicas e seus domínios, da Teoria de Wanda Horta. A elaboração do Subconjunto Terminológico da CIPE® para o cuidado à gestante durante o trabalho de parto visa contribuir para o cuidado de enfermagem através da construção e disponibilização de uma ferramenta com linguagem padronizada e unificada a fim de sustentar a prática de enfermagem baseada em evidências e favorecer a implementação do processo de enfermagem, direcionando as ações dos profissionais da área.
