Resumo: Lung cancer has been considered a public health problem since it has become one of the most commonly diagnosed neoplastic diseases and one of the most lethal in the world. This spread of lung cancer in the world population has been clearly observed in statistics and has been closely related to tobacco consumption. Cigarette smoking is considered one of the major risk factors that has led to cancer development since the 50s. Researches have shown that there is a close relationship between smoking inflammation - prothrombotic events - lung cancer. In this context, the analysis of biomarkers of inflammation is important since inflamed sites are conducive to tumor development environments. Furthermore, it is important to study the activity of ecto-nucleotide triphosphate diphosphohydrolase (E-NTPDase), ecto-5'-nucleotidase and ecto adenosine deaminase (E-ADA) enzymes which are engaged in regulating the levels of adenine nucleotides and nucleoside widely involved in inflammation and platelet aggregation. Oxidative stress is associated with the development and progression of cancer. Thus, it is extremely important to evaluate the aspects of cellular functions in a group of patients with lung cancer. The objectives of this study were i) to verify the serum levels of cytokines such as IL-6, IL-17, IL-4, IL-2, IL-10, interferon- (IFN-) and Tumor Necrosis Factor (TNF); ii) to evaluate the activity of the E-NTPDase, E-NPP, ecto-5'-nucleotidase and E-ADA platelets, and platelet aggregation; iii) to evaluate the activity of δ-aminolevulinic acid dehydratase (δ-ALA-D) erythrocyte, the levels of thiobarbituric acid reactive species (TBARS) in platelet and of reactive species (RS) in serum species; and iv) to analyze the enzymatic antioxidant profile - through the activity of superoxide dismutase (SOD) and catalase (CAT) in platelets - and non-enzymatic profile - through the content of total thiols (T-SH) and non-protein thiols (NPSH) in platelet and of the levels of vitamin C in plasma vitamin E in serum. The study group had non-small cell lung cancer and was treated at the Hematology/Oncology Section at the University Hospital of Santa Maria. The project was approved by the Ethics Committee of the Federal University of Santa Maria, CAAE (Certificate of Appreciation Presentation for Ethics) and is registered under number: 0061.0.243.000-10. Results showed that patients with lung cancer presented an increase in serum levels of IL-6, which are closely related to the development and progression of tumor, since this cytokine has proinflammatory and pro-tumor activities. The levels of IL-17 were decreased in these patients, which may be related to the advanced stage of the development of lung cancer in these subjects. Patients with lung cancer showed a decrease in E-NTPDase activity for the hydrolysis of ADP, which contributes to the expansion of procoagulant effects commonly observed in patients with neoplastic diseases.This hypothesis is strengthened by the increased platelet aggregation observed when ADP was used as the agonist process. Patients enrolled in this study showed a decrease in the CD39 expression in platelets, which is in agreement with that observed for the NTPDase activity in platelets.The studied group had an increase in the ecto-5'-nucleotidase activity and a decrease in the E-ADA activity in platelets. This suggests that the extracellular adenosine levels were elevated leading to the advance of the tumor development due to the promoting actions characteristics of adenosine such as tumor growth and of stimulating angiogenesis. Regarding the oxidative profile, the activity of δ-ALA-D enzyme was inhibited in patients with lung cancer, which may predict the occurrence of oxidative stress in these patients. This scenario can be confirmed by the elevated levels of TBARS and RS observed in patients with lung cancer. This overproduction of oxidants may also be causing the inhibition of SOD and CAT enzymes in platelets and thus their structure and the function may be compromised. Furthermore, there was a decrease in the levels of important non-enzymatic antioxidants in patients with lung cancer, showing that their beneficial actions may be occurring in an insufficient way. High levels of T-SH and possibly glutathione are implicated in the development of tumor resistance to chemotherapy. Therefore, the results described here suggest that lung cancer exerts a significant change in serum levels of IL-6 and IL-17, in the activity of the enzymes of the purinergic system in platelets, as well as in the oxidative profile of these patients. Thus, it is hoped that this work may have contributed to a better understanding of the role of the purinergic system and biomarkers of inflammation and oxidative stress in the pathophysiology of lung cancer.

Essa disseminação do câncer de pulmão na população mundial, que é claramente observada em dados estatísticos, está intimamente relacionada ao consumo de tabaco, sendo o tabagismo considerado um dos principais fatores de risco que levam ao desenvolvimento de câncer desde a década de 50. As pesquisas têm demonstrado que há uma estreita relação entre: tabagismo - processos inflamatórios - eventos pró-trombóticos - câncer de pulmão. Nesse contexto, torna-se importante a análise de biomarcadores de inflamação, já que sítios inflamados são ambientes propícios para o desenvolvimento de tumores, assim como o estudo da atividade das enzimas ecto-nucleotídeo trifosfato difosfoidrolase (E-NTPDase), ecto-5 -nucleotidase, ectonucleotídeo pirofosfatase/fosfodiesterase (E-NPP) e ecto adenosina desaminase (E-ADA), que estão envolvidas na regulação dos níveis de nucleotídeos e nucleosídeo de adenina amplamente envolvidos na inflamação e na agregação plaquetária.Trabalhos revelam que o estresse oxidativo está intimamente associado ao desenvolvimento e à progressão de neoplasias, razão pela qual torna-se extremamente relevante a avaliação desses aspectos das funções celulares em um grupo de pacientes com câncer de pulmão. Diante disso, os objetivos deste estudo foram: a) verificar os níveis séricos de citocinas como a IL-6, IL-17, IL-4, IL-2, IL-10, interferon- (INF-) e o fator de necrose tumoral (TNF); b) avaliar a atividade da E-NTPDase, da E-NPP, da ecto-5 -nucleotidase e da E-ADA em plaquetas, bem como a agregação plaquetária; c) avaliar a atividade da δ-aminolevulínico desidratase (δ-ALA-D) eritrocitária, os níveis de espécies reativas ao ácido tiobarbitúrico (TBARS) em plaquetas e de espécies reativas (ER) no soro; d) analizar o perfil antioxidante enzimático - através da atividade das enzimas superóxido dismutase (SOD) e da catalase (CAT) em plaquetas e o perfil antioxidante não enzimático - através do conteúdo de tióis totais (T-SH) e tióis não protéicos (NPSH) em plaquetas e dos níveis de vitamina C no plasma e de vitamina E no soro, em um grupo de pacientes com câncer de pulmão de não pequenas células, atendidos no setor de Hematologia/Oncologia do Hospital Universitário de Santa Maria. Quanto aos aspectos éticos, o projeto foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Santa Maria, CAAE (Certificado de Apresentação para Apreciação Ética) e está registrado sob o número: 0061.0.243.000-10. Os resultados encontrados demonstram que os pacientes com câncer de pulmão apresentaram um incremento nos níveis séricos de IL-6 os quais estão intimamente relacionados com o desenvolvimento e a progressão tumoral, visto que essa citocina apresenta atividade pró-inflamatória e pró-tumoral. Além disso, os níveis de IL-17 estão diminuídos nesses pacientes, o que pode estar relacionado com o estágio avançado de desenvolvimento do câncer de pulmão nos indivíduos estudados. Ademais, os pacientes com neoplasia pulmonar mostraram uma diminuição na atividade da E-NTPDase para a hidrólise do ADP, fato que contribui para a ampliação dos efeitos pró-coagulantes comumente observados em pacientes com doenças neoplásicas. Essa hipótese é reforçada pelo aumento da agregação plaquetária observado quando ADP foi utilizado como agonista do processo. Os pacientes envolvidos nesse estudo apresentaram uma diminuição na expressão da CD39 em plaquetas, que está de acordo com o observado para a atividade da NTPDase nas mesmas. Ainda, os pacientes com câncer de pulmão tiveram um aumento na atividade da ecto-5 -nucleotidase e uma diminuição na atividade da E-ADA em plaquetas, sugerindo que os níveis extracelulares de adenosina estão elevados nos mesmos, proporcionando avanço no desenvolvimento tumoral tendo em vista as ações promotoras de crescimento tumoral e de estímulo à angiogênese características da adenosina. Em relação ao perfil oxidativo, a atividade da enzima δ-ALA-D apresentou-se inibida em pacientes com câncer de pulmão, o que pode predizer a ocorrência de um quadro de estresse oxidativo nesses pacientes. Esse cenário pode ser confirmado pelos elevados níveis de TBARS e de ER verificados nos pacientes com neoplasia pulmonar. Essa superprodução de oxidantes também pode estar ocasionando a inibição das enzimas SOD e CAT nas plaquetas e, desse modo, a função e a estrutura das mesmas pode estar comprometida. Além disso, houve uma diminuição nos níveis de importantes antioxidantes não enzimáticos nos pacientes com câncer de pulmão, mostrando que suas ações benéficas podem estar sendo exercidas de modo insuficiente. Por outro lado, os níveis elevados de T-SH e, possivelmente, da glutationa, estariam implicados no desenvolvimento de resistência tumoral à quimioterapia. Portanto, os resultados aqui descritos sugerem que o câncer de pulmão exerce uma importante alteração nos níveis séricos de IL-6 e IL-17, na atividade das enzimas do sistema purinérgico em plaquetas, bem como no perfil oxidativo desses pacientes, podendo os parâmetros mencionados nesta pesquisa serem úteis na clínica médica, para o monitoramento da patologia.
