Resumo: Introduction: Post-bariatric abdominoplasty is the most sought-after surgery for the correction of deformities due to large weight losses. The main complication is the seroma, which may require several percutaneous punctures and sometimes results in pseudobursa. Objective: To evaluate the Scarpa fascia in the formation of the seroma in post-bariatric abdominoplasty. Methods: Forty-six female patients, post-bariatric surgery, were randomized in two groups of twenty-three patients, to perform abdominoplasty in anchor-line. In one of the groups the Scarpa fascia was maintained, and in the other group the Scarpa fascia was not maintained. A closed suction drain was maintained in the patients until de volume drained was less than 30ml in 24h. For the investigation of the seroma, an ultrasonography was performed in all the patients in the 20th postoperative period, in five regions of the abdominal wall: Epigastric, Umbilical, Hypogastric, Right Iliac Fossa, Left Iliac Fossa. The liquid found to be greater than 30 ml was considered positive for seroma. Results: When the fascia was not preserved, the average drainage was 428.9ml, when it was conserved the mean was 249.1ml (p=0.003). Regarding the length of stay of the drain; the average was 5.4 days when it was not preserved, and it was 4.2 days when preserved (p=0.012). When the fascia was removed the mean volume of the liquid on ultrasonography was 21.6 ml; when preserved the mean was 16.8ml (p=0.809). Conclusion: The maintenance of the Scarpa fascia reduced the amount drained as well as the time of drainage use, but did not reduce the incidence of seroma.

Conclusion: The maintenance of the Scarpa fascia reduced the amount drained as well as the time of drainage use, but did not reduce the incidence of seroma.Introdução: A abdominoplastia é a primeira opção cirúrgica para a correção de deformidades decorrentes de grandes perdas ponderais, tendo como principal complicação o seroma, que pode exigir várias punções percutâneas e, algumas vezes, resultar em pseudobursa. Objetivo: Avaliar a fáscia de Scarpa na formação do seroma nas abdominoplastias pós-bariátricas em âncora. Método: Realizou-se 46 abdominoplastias em ancora pós-bariátricas, em pacientes do gênero feminino distribuídas, aleatoriamente, em dois grupos com 23 pacientes cada: Um grupo foi mantida a fáscia de Scarpa e outro grupo não foi mantida. As pacientes foram mantidas com dreno de sucção fechado até que o volume drenado fosse inferior a 30ml em 24h. Investigou-se a presença de seroma com a ultrassonografia, em todas as pacientes no 20º dia pós-operatório, em cinco regiões da parede abdominal: Epigastrio, Umbilical, Hipogastrio, Fossa Ilíaca Direita, Fossa Ilíaca Esquerda. Considerou-se positivo para seroma quando existia volume líquido superior a 30ml. Resultados: Quando não foi preservada a fáscia, a média de drenagem foi de 428,9ml. Com a sua preservação, a média foi de 249,1ml (p=0,003). Em relação ao tempo de permanência a média foi de 5,4 dias quando não se preservou, e de 4,2 dias quando preservada (p=0,012). Quando retirada a fáscia, a média do volume do líquido, na ultrassonografia, foi de 21,6ml; quando preservada, obteve-se a média de 16,8ml (p=0,809).
