Transtornos mentais comuns e qualidade de vida entre profissionais de saúde em um hospital de ensino.

Resumo: Common Mental Disorders (CMD) present high prevalence in several general populations and workers, mostly the health professionals. Mental Disorders can cause social consequences, reduction of labor productivity and psychic suffering. This study was aimed at analyzing the relation between Common Mental Disorders and Quality of Life among health professionals of a school hospital. It is an observational, transversal, quantitative study conducted with 359 health professionals. The project was approved by the Committee of Ethics in Research with Human Beings of University Federal of Triangulo Mineiro, protocol nr 2505. The participants answered an instrument with professional and sociodemographic variables; another to check the prevalence of CMD (SRQ-20); and another one to evaluate Quality of Life (WHOQOL-bref). The data were typed twice in Microsoft Excel Program®, and exported to Statistical Package for Social Sciences, version 20.0 for analysis. It was observed the predominance of female sex (77.2%), aged between 30 to 39 years old (35.9%); income between 2 and 4 minimum wages (31.5%). As to professional profile, (54.9%) worked during the day shift; (67.1%) mentioned only one employment contract; (28.4%) had 1 to 5 years of working experience. Among the health professionals, it was observed an overall prevalence (27.9%) for CMD. Among the symptoms evaluated the group of symptoms of mood depressed/anxious was predominant, specially the complaints about feeling nervous, tense or worried (64.6%). The bivariate analysis showed that the prevalence of CMD was (33.6%) in nursing staff, (17.9%) among other health professionals and (9.1%) for physicians. The logistic regression analysis revealed that among health professionals, women had 2.36 times higher chance (1.09 to 5.10) of having CMD and among those aged under 39 years old were 2.21 higher (1.31 to 3.71) for tracking CMD to be positive, even after adjustment for variables identified as potentially confusing ones. The domain of the WHOQOL-bref that showed the highest score was the physical (68.98 points) and the lowest score was environmental (58.17 points). People with higher family income had higher scores for all QOL domains. As to gender, the scores of the physical, psychological and environmental domains of QOL experienced by the males were higher than females of the group. Regarding the shift, for the physical domain other shifts had the highest score of QOL compared to the night shift. The results of multiple linear regression analysis showed that presenting CMD and being part of the nursing category negatively impacted the Physical and Environment domains of QOL, however, being female and having higher family income increases positively the environmental domain of QOL. The indicative of CMD was the predictor that most negatively impacted in all QOL domains. The results of this study indicate that the presence of CMD negatively affects QOL and it is necessary to propose measures to promote the health of health professionals.

Os transtornos mentais podem ocasionar consequências sociais, redução na produtividade laboral e sofrimento psíquico. Este estudo objetivou analisar a relação de Transtornos Mentais Comuns e Qualidade de Vida entre os profissionais de saúde de um hospital de ensino. Trata-se de um estudo observacional, transversal com abordagem quantitativa, realizado entre 359 profissionais de saúde. O projeto foi aprovado pelo Comitê de Ética em Pesquisa em Seres Humanos da Universidade Federal do Triângulo Mineiro, protocolo nº 2505. Os participantes responderam um instrumento contendo variáveis sociodemográficas e profissionais; outro, para verificar a prevalência de TMC (SRQ-20); e outro, para avaliação da Qualidade de Vida (WHOQOL-bref). Os dados foram duplamente digitados no programa Microsoft Excel®, e importados ao Statistical Package for Social Sciences, versão 20.0, para análise. Foi observado predomínio do sexo feminino (77,2%), com idade entre 30 e 39 anos (35,9%); com renda entre 2 e 4 salários mínimos (31,5%). Quanto ao perfil profissional, (54,9%) pertenciam ao turno diurno; (67,1%) referiram apenas um vínculo empregatício; (28,4%) tinham de 1 a 5 anos na profissão. Entre os profissionais de saúde detectou-se uma prevalência geral de (27,9%) para TMC. Dentre os sintomas avaliados predominou o grupo de sintomas humor depressivo/ansioso, com destaque para a queixa sentir-se nervoso, tenso ou preocupado (64,6%). A análise bivariada demonstrou que a prevalência de TMC foi de (33,6%) na equipe de enfermagem, (17,9%) entre outros profissionais de saúde e (9,1%) em médicos. A análise de regressão logística revelou que entre os profissionais de saúde, as mulheres tiveram chance 2,36 vezes mais elevada (1,09- 5,10) de apresentar TMC e entre aqueles com idade inferior a 39 anos apresentaram chance 2,21 vezes mais elevada (1,31-3,71) de o rastreamento para TMC ser positivo, mesmo após o ajuste por variáveis identificadas como potenciais confundidoras. O domínio do WHOQOL-bref que apresentou o maior escore foi o físico (68,98 pontos) e o menor escore foi o ambiental (58,17 pontos). Pessoas com renda familiar mais alta apresentaram escores mais elevados de QV para todos os domínios. Quanto à variável sexo, os escores dos domínios físico, psicológico e ambiental de QV obtidos no grupo do sexo masculino foram maiores que os do grupo do sexo feminino. Em relação ao turno de trabalho, para o domínio físico outros turnos apresentou o maior escore de QV quando comparado ao turno noturno. Os resultados da análise de regressão linear múltipla demonstram que apresentar TMC e fazer parte da categoria de enfermagem impactou negativamente nos domínios físico e ambiental de QV, no entanto, ter maior renda familiar e ser do sexo feminino aumenta positivamente o domínio ambiental da QV. O indicativo de TMC foi o preditor que mais impactou negativamente em todos os domínios de QV.
