Qualidade de vida e toxicidade aguda por radiação em pacientes com câncer de colo do útero, de endométrio e de mama submetidas à radioterapia

Resumo: Treatments for cancer of the cervix, endometrial cancer and breast cancer, vary and are established after staging. The radiation reaches all types of biological material, so, it also affects normal tissues. Adverse effects to the adjacent healthy tissues that were irradiated are classified as acute toxicities and directly affect the levels of quality of life (QoL) of these patients. The overall objective of this study was to assess quality of life and degree of acute radiation toxicity in patients with cervical cancer, breast cancer and endometrial cancer in radiotherapy. Quantitative, descriptive, prospective, longitudinal study was conducted at the Radiotherapy Service of the General Hospital in Federal University of Triangulo Mineiro (RT/HC/UFTM). To collect data, were used three instruments: one for knowledge of the sociodemographic and clinical profile of patients, the Acute Radiation Morbidity Scoring Criteria of Radiation Therapy Oncology Group (RTOG) and the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire version 3.0. Data were processed and analyzed using Statistical Package for Social Science (SPSS) version 20. In the analysis was used simple frequency distribution, mean comparison, and Spearman correlation. The group was consisted of 16 women aged 34 to 75 years old, coming from Uberaba (Brazil), incomplete primary education and a home, with personal income up to the minimum Brazilian salary. The degree of toxicity was higher and in less time for patients with cervical cancer and endometrial cancer and the number of women who had grade three was higher in this group. On the other hand, was largest the number of women who had grade one and higher the length of time between the grade zero and the advancing degrees of toxicity. The QoL presented by the group was considered good. The most affected function was emotional function, the symptom with the higher average was fatigue and the symptom that reached higher maximum value was diarrhea. The lack of statistical difference between the measures of QoL scores obtained in the first and last week may indicate that radiotherapy did not affect the QoL of these women.

A radiação atinge todo tipo de material biológico, portanto, atinge também tecidos normais. Os efeitos adversos nos tecidos sadios adjacentes à área irradiada são classificados como toxicidades agudas e afetam diretamente os níveis de qualidade de vida (QV) das pacientes. O objetivo geral deste estudo foi avaliar a qualidade de vida e o grau de toxicidade aguda por radiação das pacientes portadoras de CCU, CA de mama e CA de endométrio, em tratamento radioterápico. Trata-se de estudo quantitativo, descritivo, prospectivo e longitudinal, desenvolvido no serviço de Radioterapia do Hospital de Clínicas da Universidade Federal do Triângulo Mineiro (RT/HC/UFTM). Para a coleta de dados, utilizaram-se três instrumentos: um para conhecimento do perfil sociodemográfico e clínico das pacientes, o Critério de Escore para Morbidade Aguda por Radiação (CEMAR) do Radiation Therapy Oncology Group (RTOG) e o European Organization for Research and Treatment of Cancer Quality of Life Questionnaire versão 3.0. Os dados foram processados e analisados no programa Statistical Package for Social Science (SPSS) versão 20. Na análise, utilizaram-se distribuição de frequência simples, comparação de médias e correlação de Spearman. O grupo foi composto por 16 mulheres com idade entre 34 e 75 anos, procedentes de Uberaba, com ensino fundamental incompleto, do lar, com renda individual de até um salário-mínimo (SM). A maior parte realizava tratamento para CA de mama, apresentava estadiamento inicial, realizou cirurgia antes da radioterapia (RT), recebeu dose total de radiação entre 5001 e 6000 cGy, fracionada em 33 sessões de 180 cGy. O grau de toxicidade foi maior e em menor tempo para as pacientes com CCU e CA de endométrio, e a quantidade de mulheres a apresentar grau três foi maior nesse grupo. Em contrapartida, foi maior o número de mulheres a apresentar grau um e maior o tempo entre o grau zero e o avanço dos graus de toxicidade. A QV apresentada pelo grupo foi considerada boa. A função mais afetada foi a emocional, o sintoma com maior média foi a fadiga e o que atingiu maior valor máximo foi a diarreia. Os graus de toxicidade apresentaram correlação, em algum momento do tratamento, com a função social e os relatos de diarreia.
