Estudo dos fatores de risco cardiometabólicos em mulheres com Tireoidite Crônica Autoimune

Resumo: Chronic autoimmune thyroiditis (CAT) is the most common cause of hypothyroidism in adults, especially in women. The main clinical manifestations are the signs and symptoms of hypothyroidism. Metabolic Syndrome (MS) is a complex interrelated set of risk factors for cardiovascular disease and diabetes. These factors include hyperglycemia, hypertension, elevated triglyceride levels, low HDL-c, and accumulation of abdominal fat. Estimating the risk of atherosclerotic disease results from the sum of the risk associated with each of the risk factors plus the potentiation caused by synergisms between some of these factors. The thyroid and SM binomials are highlighted, since dysfunction of the thyroid gland alters important metabolic pathways resulting in the establishment of the constituent elements of MS. The main objective of this study was to study the main cardiometabolic risk factors, clinical and laboratorial, present in women with CAT treated with levothyroxine and therefore euthyroid. This is a cross-sectional, analytical study with a quantitative approach. Data were collected at the UFTM Endocrinology outpatient clinic using an instrument that included socioeconomic, anthropometric, clinical and laboratory data. Twenty women with CAT and 20 women without thyroid disease aged between 20 and 45 years participated in the study. Categorical variables were analyzed using absolute and relative frequency distributions. A comparative analysis of cardiometabolic risk factors between the affected group and the control group was performed. Also comparative analysis of frequencies of occurrences of cardiometabolic risk factors between the CAT and normal groups; Besides correlations between numerical clinical and laboratory variables. The level of significance of the inferential procedures was 5%. It was observed that the women with CAT presented values of BMI compatible with overweight or obesity in 70% of the cases and abdominal circumference above 80 cm in 70%, being statistically higher in relation to the control group. Regarding the metabolic profile, it was observed that the means of fasting glycemia concentrations were significantly higher in the group with CAT than in the control group, and the same was observed in relation to the HOMA index. Regarding the lipid profile, it was verified that there was no significant difference between the groups, although more people in the CAT group presented borderline values, demonstrating the effectiveness of hormone replacement therapy. Overall risk scorewas significantly higher in patients with CAT than in the control group, presenting an intermediate risk for cardiovascular disease, while the control group presented low risk. By definition and conventional criteria adopted, 20% of patients with CAT versus 7% of the control group had 3 or more cardiometabolic risk factors. It was concluded from these results that patients with CAT had significantly higher fasting glycemia and HOMA-IR levels than in the control group; Although the basal insulin concentrations did not present difference between the two groups, these data suggest the presence of insulin resistance. Whether it is due to CAT or overweight / obesity, the design of the present study did not allow the conclusion. To do so, a future study should necessarily select patients with CAT and normal controls with normal BMI.

Las principales manifestaciones clínicas son los signos y síntomas de hipotiroidismo. El Síndrome Metabólico (SM) es una compleja interrelación de factores de riesgo para enfermedades cardiovasculares y diabetes. Estos factores incluyen hiperglucemia, hipertensión, concentraciones elevadas de triglicéridos, HDL-c bajo y acumulación de grasa abdominal. La estimación del riesgo de enfermedad aterosclerótica resulta de la suma del riesgo asociado a cada uno de los factores de riesgo más la potencialización causada por sinergismos entre algunos de estos factores. El binomio tiroidea y SM se hace evidente, ya que la disfunción de la glándula tiroidea altera importantes vías metabólicas, resultando en la instalación de los elementos constituyentes de la SM. El principal objetivo de este trabajo fue estudiar los principales factores de riesgo cardiometabólicos, clínicos y de laboratorio, presentes en mujeres con TCA tratadas con levotiroxina y por lo tanto eutiriasideas. Se trata de un estudio transversal, analítico, con abordaje cuantitativo. Los datos fueron recolectados en el ambulatorio de Endocrinología de la UFTM utilizando un instrumento que contemplaba los datos socioeconómicos, antropométricos, clínicos y de laboratorio. Participaron de la investigación 20 mujeres con TCA y 20 mujeres sin enfermedad tiroidea con edad entre 20 y 45 años. Las variables categóricas se analizaron empleando distribuciones de frecuencia absoluta y relativa. Se realizó un análisis comparativo de los factores de riesgo cardiometabólicos entre el grupo afectado y el grupo control. Y también análisis comparativo de las frecuencias de los sucesos de los factores de riesgo cardiometabólicos entre los grupos TCA y normal; Además de correlaciones entre variables numéricas clínicas y de laboratorio. El nivel de significancia de los procedimientos inferenciales fue del 5%. En el caso de las mujeres con TCA presentaron valores de IMC compatibles con sobrepeso u obesidad en el 70% de los casos y circunferencia abdominal por encima de 80 cm en un 70%, siendo estadísticamente mayor en relación al grupo control. En relación al perfil metabólico se notó que las medias de las concentraciones de la glucemia de ayuno fueron significativamente mayores en el grupo con TCA que en elgrupo control, y lo mismo fue observado en relación al índice HOMA. En cuanto al perfil lipídico, se verificó que no hubo diferencia significativa entre los grupos, aunque mayor número de personas en el grupo con TCA presentaron valores limítrofes, demostrando la eficacia del tratamiento de reposición hormonal. La puntuación de Riesgo global fue significativamente mayor en los pacientes con TCA en relación al grupo control, al presentar riesgo intermedio para enfermedad cardiovascular, mientras que el grupo control presentó bajo riesgo. Por definición y criterios convencionales adoptados, el 20% de los pacientes con TCA contra el 7% del grupo control presentaron 3 o más factores de riesgo cardiometabólicos. Se concluye, por esos resultados, que pacientes con TCA presentaron concentraciones de glucemia de ayuno y índice HOMA-IR significativamente mayor que en el grupo control; Aunque las concentraciones basales de insulina no presentaron diferencias entre los dos grupos, estos datos sugieren la presencia de resistencia insulínica. Si la misma se debe a la TCA o al sobrepeso / obesidad, el diseño del presente estudio no permitió concluir. Para ello, un estudio futuro deberá necesariamente seleccionar pacientes con TCA y controles normales con IMC normal.A Tireoidite Crônica Autoimune (TCA) é a causa mais comum de hipotireoidismo em adultos, sobretudo em mulheres. As principais manifestações clínicas são os sinais e sintomas de hipotireoidismo. A Síndrome Metabólica (SM) é uma complexa inter-relação de fatores de risco para doenças cardiovasculares e diabetes. Esses fatores incluem hiperglicemia, hipertensão, concentrações elevadas de triglicerídeos, HDL-c baixo e acúmulo de gordura abdominal. A estimativa do risco de doença aterosclerótica resulta da somatória do risco associado a cada um dos fatores de risco mais a potencialização causada por sinergismos entre alguns desses fatores. O binômio tireoide e SM ganha destaque, uma vez que disfunção da glândula tireoidiana altera importantes vias metabólicas resultando na instalação dos elementos constituintes da SM. O principal objetivo deste trabalho foi estudar os principais fatores de risco cardiometabólicos, clínicos e laboratoriais, presentes em mulheres com TCA tratadas com levotiroxina e portanto eutireoideas. Trata-se de um estudo transversal, analítico, com abordagem quantitativa. Os dados foram colhidos no ambulatório de Endocrinologia da UFTM utilizando um instrumento que contemplava os dados socioeconômicos, antropométricos, clínicos e laboratoriais. Participaram da pesquisa 20 mulheres com TCA e 20 mulheres sem doença tireoidiana com idade entre 20 e 45 anos. As variáveis categóricas foram analisadas empregando-se distribuições de frequência absoluta e relativa. Foi realizada análise comparativa dos fatores de risco cardiometabólicos entre o grupo afetado e grupo controle. E também análise comparativa das frequências das ocorrências dos fatores de risco cardiometabólicos entre os grupos TCA e normal; além de correlações entre variáveis numéricas clínicas e laboratoriais. O nível de significância dos procedimentos inferenciais foi de 5%. Avaliando os índices de adiposidade notou-se que as mulheres com TCA apresentaram valores de IMC compatíveis com sobrepeso ou obesidade em 70% dos casos e circunferência abdominal acima de 80 cm em 70%, sendo estatisticamente maior em relação ao grupo controle. Em relação ao perfil metabólico notou-se que as médias das concentrações da glicemia de jejum foram significativamente maiores no grupo com TCA que no grupo controle, e o mesmo foi observado em relação ao índice HOMA. Quanto ao perfil lipídico, verificou-se que não houve diferença significativa entre os grupos, ainda que maior númerode pessoas no grupo com TCA tenham apresentado valores limítrofes, demonstrando a eficácia do tratamento de reposição hormonal. Escore de Risco global foi significativamente maior nos pacientes com TCA em relação ao grupo controle, ao apresentar risco intermediário para doença cardiovascular, enquanto o grupo controle apresentou baixo risco. Por definição e critérios convencionais adotados, 20% dos pacientes com TCA contra 7% do grupo controle apresentaram 3 ou mais fatores de risco cardiometabólicos. Conclui-se, por esses resultados, que pacientes com TCA apresentaram concentrações de glicemia de jejum e índice HOMA-IR significantemente maiores que no grupo controle; ainda que as concentrações basais de insulina não tenham apresentado diferença entre os dois grupos, estes dados sugerem a presença de resistência insulínica. Se a mesma se deve à TCA ou ao sobrepeso/obesidade, o desenho do presente estudo não permitiu concluir. description Chronic autoimmune thyroiditis (CAT) is the most common cause of hypothyroidism in adults, especially in women. The main clinical manifestations are the signs and symptoms of hypothyroidism. Metabolic Syndrome (MS) is a complex interrelated set of risk factors for cardiovascular disease and diabetes. These factors include hyperglycemia, hypertension, elevated triglyceride levels, low HDL-c, and accumulation of abdominal fat. Estimating the risk of atherosclerotic disease results from the sum of the risk associated with each of the risk factors plus the potentiation caused by synergisms between some of these factors. The thyroid and SM binomials are highlighted, since dysfunction of the thyroid gland alters important metabolic pathways resulting in the establishment of the constituent elements of MS. The main objective of this study was to study the main cardiometabolic risk factors, clinical and laboratorial, present in women with CAT treated with levothyroxine and therefore euthyroid. This is a cross-sectional, analytical study with a quantitative approach. Data were collected at the UFTM Endocrinology outpatient clinic using an instrument that included socioeconomic, anthropometric, clinical and laboratory data. Twenty women with CAT and 20 women without thyroid disease aged between 20 and 45 years participated in the study. Categorical variables were analyzed using absolute and relative frequency distributions. A comparative analysis of cardiometabolic risk factors between the affected group and the control group was performed. Also comparative analysis of frequencies of occurrences of cardiometabolic risk factors between the CAT and normal groups; Besides correlations between numerical clinical and laboratory variables. The level of significance of the inferential procedures was 5%. It was observed that the women with CAT presented values of BMI compatible with overweight or obesity in 70% of the cases and abdominal circumference above 80 cm in 70%, being statistically higher in relation to the control group. Regarding the metabolic profile, it was observed that the means of fasting glycemia concentrations were significantly higher in the group with CAT than in the control group, and the same was observed in relation to the HOMA index. Regarding the lipid profile, it was verified that there was no significant difference between the groups, although more people in the CAT group presented borderline values, demonstrating the effectiveness of hormone replacement therapy. Overall risk scorewas significantly higher in patients with CAT than in the control group, presenting an intermediate risk for cardiovascular disease, while the control group presented low risk. By definition and conventional criteria adopted, 20% of patients with CAT versus 7% of the control group had 3 or more cardiometabolic risk factors. It was concluded from these results that patients with CAT had significantly higher fasting glycemia and HOMA-IR levels than in the control group; Although the basal insulin concentrations did not present difference between the two groups, these data suggest the presence of insulin resistance. Whether it is due to CAT or overweight / obesity, the design of the present study did not allow the conclusion. To do so, a future study should necessarily select patients with CAT and normal controls with normal BMI.
