Resumo: Introduction: Elderly patients with cancer are at high risk of malnutrition. The Patient Generated Subjective Global Assessment (PG-SGA) has been considered the most suitable method for cancer patients, although anthropometry is still widely used in clinical practice. Because it is a subjective method, recently validated for the Brazilian population, comparative studies between different methods are needed. Besides the nutritional diagnosis, the identification of factors associated with malnutrition is an important aspect in the management of elderly patients with cancer, including the investigation of the presence of depression and cognitive impairment. Objectives: To analyze the clinical, sociodemographic and nutritional profile, the factors associated with malnutrition and compare the nutritional diagnosis by PG-SGA with objective anthropometric measures in elderly patients undergoing cancer treatment. Methods: A cross-sectional study was carried out with 96 elderly patients undergoing cancer treatment. The procedures included the application of a sociodemographic and health questionnaire, assessment of nutritional status by PG-SGA and determination of measures and anthropometric indices: weight, height, Body Mass Index, mid-upper arm circumference, arm muscle circumference, arm muscle area, corrected arm muscle area, arm fat area, calf circumference, waist circumference, hip circumference, waist to hip ratio and triceps skinfold. The current food consumption was investigated by a 24-Hour Recall, the presence of depression was assessed by the Geriatric Depression Scale with 15 items (GDS-15) and cognitive function by the Mini Mental State Examination (MMSE). Statistical analysis involved descriptive measures, univariate and multivariate analysis. The Poisson regression with robust variance adjustment was used to identify the factors independently associated with malnutrition. Comparison between methods of nutritional evaluation was performed by Spearman correlation and Kappa coefficient adjusted prevalence. In all statistical tests, the significance level for rejection of the null hypothesis was α = 0,05. Results: The sample was equitable in relation to gender, with a mean age of 70,6 years (SD = 7,8 years) and predominance of individuals with advanced disease. The PG-SGA identified 56,2% of elderly normal weight, 29,2% moderately malnourished or suspected malnutrition and 14,6% with severe malnutrition. Of elderly evaluated, 47,9% needed critical nutritional intervention. Anthropometric parameters differ according to the subjective classification of PG-SGA, with significant correlation between subjective and objective methods. The prevalence of malnutrition ranged from 43,8% to 61,4%, depending on the method used, with greater agreement between the diagnosis provided by PG-SGA with the Body Mass Index (kappa = 0,54, CI: 0,347-0,648) Cognitive impairment was identified in 39,6% and the presence of depression in 17,7%, with higher scores for females on the GDS-15 (p = 0,017). In the multivariate analysis were independently associated with malnutrition the functional impairment (PR: 3,40, CI: 1,23-9,45), the presence of two or more symptoms with nutritional impact (PR: 3,22 CI: 1,03-10,10 ) and current treatment by chemotherapy (PR: 2,96, CI: 1,16-7,56). Conclusions: Elderly patients in cancer treatment showed high prevalence of malnutrition and need for critical nutritional intervention, reiterating the importance of nutrition as central action in the management of these patients. The ASG-PPP significantly correlated with anthropometric and food intake variables, confirming his recomendation as the preferred method for the nutritional assessment of elderly patients with cancer. About two to five assessed had cognitive impairment and depression was identified in a every six elderly, with higher scores on the depression scale for females. The functional impairment, the presence of two or more symptoms with nutritional impact and current treatment by chemotherapy were independently associated with malnutrition. These conditions must be investigated effectively in the evaluation and intervention in these individuals.

A Avaliação Subjetiva Global Produzida pelo Paciente (ASG-PPP) tem sido considerada o método mais adequado para a avaliação nutricional de pacientes oncológicos, embora a antropometria ainda seja largamente utilizada na prática clínica. Por ser um método subjetivo e recentemente validado para a população brasileira, estudos comparativos entre os diferentes métodos são necessários. Além do diagnóstico nutricional, a identificação dos fatores associados à desnutrição são aspectos relevantes na abordagem do idoso com câncer, incluindo a investigação da presença de depressão e de déficit cognitivo. Objetivos: Analisar o perfil clínico, sociodemográfico, nutricional, os fatores associados à desnutrição e comparar o diagnóstico nutricional obtido pela ASG-PPP com medidas antropométricas objetivas em idosos em tratamento oncológico. Metodologia: Foi realizado um estudo transversal com 96 idosos em tratamento em um centro oncológico. Os procedimentos adotados incluíram a aplicação de um questionário sociodemográfico e de saúde, avaliação do estado nutricional pela ASGPPP e determinação de medidas e índices antropométricos: peso, estatura, Índice de Massa Corporal, perímetro do braço, circunferência muscular do braço, área muscular do braço, área muscular do braço corrigida, área adiposa do braço, perímetro da panturrilha, perímetro da cintura, perímetro do quadril, relação cintura-quadril e prega cutânea tricipital. O consumo alimentar atual foi investigado por meio de um Recordatório de 24 horas, a presença de depressão foi avaliada pela Escala de Depressão Geriátrica versão com 15 itens (GDS-15) e a função cognitiva pelo do Mini-Exame do Estado Mental (MEEM). A análise estatística envolveu medidas descritivas, análise univariada e multivariada. Foi utilizada a regressão de Poisson com ajuste de variância robusta para identificar os fatores independentemente associados à desnutrição. A comparação entre os métodos de avaliação nutricional foi feita por meio da correlação de Spearman e do cálculo do coeficiente Kappa ajustado à prevalência. Em todos os testes estatísticos, o nível de significância para rejeição da hipótese de nulidade foi α = 0,05. Resultados: A amostra foi equitativa em relação ao sexo, com idade média de 70,6 anos (DP = 7,8 anos) e predomínio de indivíduos com doença em estágio avançado. Por meio da ASG-PPP identificou-se 56,2% de idosos eutróficos, 29,2% com desnutrição moderada ou suspeita de desnutrição e 14,6% com desnutrição grave. Dos idosos avaliados, 47,9% necessitavam de intervenção nutricional crítica. Os parâmetros antropométricos apresentaram diferenças segundo a classificação subjetiva da ASG-PPP, com correlação significativa entre os métodos objetivos e subjetivo. A prevalência de desnutrição variou de 43,8% a 61,4%, dependendo do método utilizado, com maior concordância entre o diagnóstico fornecido pela ASG-PPP com o Índice de Massa Corporal (kappa = 0,54; IC: 0,347-0,648). O déficit cognitivo foi identificado em 39,6% e a presença de depressão em 17,7% dos avaliados, com maior pontuação para o sexo feminino na GDS-15 (p=0,017). Na análise multivariada estiveram associados à desnutrição nos idosos o déficit funcional (RP: 3,40; IC: 1,23-9,45), a presença de dois ou mais sintomas de impacto nutricional (RP: 3,22; IC: 1,03-10,10) e o tratamento atual por quimioterapia (RP: 2,96; IC: 1,16-7,56). Conclusões: Os idosos em tratamento oncológico apresentaram elevada prevalência de desnutrição e de necessidade de intervenção nutricional crítica, reiterando a importância do suporte nutricional como ação central no cuidado ao indivíduo com câncer. A ASG-PPP apresentou correlação significativa com as medidas antropométricas e de consumo alimentar, confirmando-se sua indicação como método preferencial para a avaliação nutricional de idosos com câncer. Cerca de dois a cada cinco avaliados apresentaram déficit cognitivo e a depressão foi identificada em um de cada seis idosos, com maior pontuação na escala de depressão para o sexo feminino.
