A integralidade do cuidado em saúde sexual e reprodutiva: um estudo de caso sobre profissionais do Programa de Saúde da Família de Córrego Fundo- MG

Resumo: This research has a qualitative nature and aimed, through a case study, examining the notions and practices of professionals of Family Health Program - FHP regarding sexual and reproductive health, highlighting the extent to which these notions and practices are approaching or moving away from the principle of comprehensive care. It starts from the point that although public policies advocate that health care should be done comprehensively and addressing the individual as a holistic being, this is still a challenge to be achieved by health professionals. The survey data were constructed through primary sources, which are these: interviews, direct observation and interviews with health professionals, in addition to secondary sources: data from the Information System for Primary Care - SIAB, the Brazilian Institute of Geography and Statistics - IBGE and documents from the Brazilian Health Ministry, that addressed the Program for Integral Attention to Women's Health - PAISM, the Unified Health System - SUS and FHP. The research was conducted in the city of Córrego Fundo-MG, which has 5.821 inhabitants, between the months August to October 2010. The data were analyzed whithin the following categories: sex, sexuality, gender and body and revealed that, despite the policies of women's health and family health have also incorporated into their legal documents, the gender perspective, the main focus of attention to sexual and reproductive health, in the PSFs studied, is the figure of the woman, which characterizes a situation of men exclusion. It was also identified that in the field of sexual and reproduction health, the multidisciplinary team recognizes that the comprehensive approach does not exist in the health practices carried out by them, and that the realization of this principle is far from being achieved. Among the difficulties related to effective comprehensive care in health practices, it was mentioned: lack of time for a broader approach on individuals health due to bureaucracy in filling out worksheets and due to charging for productivity; the work overcharge of the Community Health Agents ACSs; the lack of team commitment for solving the population health problems; a misunderstand by the users about the preventive purpose of PSF; health practices directed to curative actions and the lack of professional training. Regarding to sexual and reproductive health, the interviewees reported that the major difficulties in developing actions in this field are due to shame, the silence of women, the fragility of the professional relationship and due also to the lack of professional training for working on these issues. From these results, we emphasize the urgent need for capacity and training programs for health professionals involved in the care process, especially for the ACSs. We believe that the proposal of a program of continued education can really help the studied family health teams to understand and better organize the care process, aiming at a better resolution of health actions, professional integration and, consequently, the integration of health actions.

Parte-se do ponto de que, embora as políticas públicas preconizem que o atendimento de saúde deva ser feito de forma integral e contemple o indivíduo como um ser holístico, esta prática ainda é um desafio a ser alcançado pelos profissionais de saúde. Os dados da pesquisa foram construídos através de fontes primárias, quais sejam: entrevistas, observação direta e conversas com os profissionais da saúde, além de fontes secundárias: dados do Sistema de Informação da Atenção Básica SIAB, do Instituto Brasileiro de Geografia e Estatística IBGE e documentos do Ministério da Saúde, que abordavam o Programa de Atenção Integral à Saúde da Mulher PAISM; o Sistema Único de Saúde SUS; e o PSF. A pesquisa empírica foi realizada no Município de Córrego Fundo, MG, que tem 5.821 habitantes, entre os meses de agosto e outubro de 2010. Os dados foram analisados à luz das categorias sexo, sexualidade, corpo e gênero e revelaram que, apesar de as políticas de saúde da mulher e também de saúde da família terem incorporado em seus documentos legais a perspectiva de gênero, o foco principal da atenção à saúde sexual e reprodutiva, nos PSFs pesquisados, está na figura da mulher, o que caracteriza uma situação de exclusão dos homens. Identificou-se que, em se tratando das ações de saúde nos campos sexual e reprodutivo, a equipe multiprofissional reconhece que a integralidade inexiste nas práticas de saúde por ela realizada e que a efetivação desse princípio está longe de ser alcançada. Entre as dificuldades mencionadas para a efetivação da integralidade do cuidado nas práticas de saúde, destacam-se: a falta de tempo hábil para uma abordagem mais ampla sobre as condições de saúde dos indivíduos, devido à burocracia no preenchimento de planilhas e à cobrança por produtividade; a sobrecarga de trabalho dos Agentes Comunitários de Saúde ACSs; a falta de comprometimento da equipe para resolver os problemas de saúde da população; a não compreensão, por parte dos usuários, sobre a proposta preventiva do PSF; as práticas de saúde voltadas para o curativismo; e a falta de capacitação profissional. Em se tratando da saúde sexual e reprodutiva, as entrevistadas relataram que as principais dificuldades para desenvolver ações nesse campo ocorrem devido à vergonha, ao silêncio das mulheres, à fragilidade do vínculo profissional e também à falta de capacitação profissional para trabalhar essas questões. A partir dos resultados, ressalta-se a urgente necessidade de capacitação e treinamento para as profissionais de saúde envolvidas no processo de cuidar, principalmente para as ACSs.
