A maioria dos pacientes apresenta tumor localmente avançado ao diagnostico, e para eles é preconizada a terapêutica multimodal com cirurgia, radioterapia e quimioterapia no Sistema Único de Saúde (SUS). Os esquemas de indução com taxano (docetaxel ou paclitaxel), cisplatina (CDDP) e 5fluorouracil (5-FU), denominado TPF, e com taxano (docetaxel ou paclitaxel) e cisplatina (CDDP), denominado TP, seguidos por RT com CDDP ou carboplatina (CARBO) são utilizados em nossa Instituição, mas desconhecemos os desfechos obtidos por nossos pacientes. Objetivo: Obter as taxas de resposta, toxicidades e a sobrevida livre de evento (SLE) e a sobrevida global (SG) de pacientes tratados com TPF e TP, de forma retrospectiva (01/01/2015 a 31/12/2019), a fim de estabelecer um protocolo de tratamento para esses pacientes no Hospital de Clínicas da Universidade Estadual de Campinas (HC/UNICAMP). Métodos: Os dados demográficos, clínico patológicos e os desfechos dos tratamentos foram obtidos dos prontuários médicos pelo pesquisador responsável pelo estudo. As diferenças entre grupos foram avaliadas pelo teste de Fisher ou qui-quadrado, regressão logística múltipla, Kaplan-Meier/log rank e análises univariada e multivariada de Cox. Resultados: Não foram observadas diferenças clínico patológicas dos 55 pacientes tratados com TPF (n=32) ou com TP (n=23). Observamos maior taxa de resposta (60,9% versus 16,1%, p=0,009) e maior SLE (66,6% versus 49%, p=0,014) em pacientes tratados com TP do que nos tratados com o TPF. Vinte e cinco de 32 (78%) pacientes sob regime TPF e 12 de 23 (52%) pacientes do grupo TP receberam ciclos de QT com intervalo igual ou maior a 28 dias (p=0.042). Não foram observadas nos números de ciclos de QT e de toxicidades aos esquemas. Conclusões: 1) Observamos desfechos melhores com TP do que com o TPF, possivelmente por atrasos na administração do TPF, e 2) Considerando as dificuldades para infusão contínua de 5-FU e os resultados obtidos nesse estudo, propomos tratar os pacientes com CCECP localmente avançado e com boa condição clínica (ECOG 0-2) do HC/UNICAMP com TPAbstract: Introduction: Squamous cell carcinoma of the head and neck (HNSCC) is the seventh most common type of cancer in the world and the fifth most common type in Brazil, and 50% of carriers evolve to death. Most patients with HNSCC have a locally advanced tumor at diagnosis, and multimodal therapy with surgery, radiotherapy and chemotherapy is recommended for them in the Unified Health System (SUS). Induction regimens with taxane (docetaxel or paclitaxel), cisplatin (CDDP) and 5-fluorouracil (5-FU), called TPF, and with taxane (docetaxel or paclitaxel) and cisplatin (CDDP), called TP, followed by RT with CDDP or carboplatin are used in our institution, but the outcomes obtained by our patients are not known. Objective: To obtain the response rates, disease-free survival (DFS) and overall survival (OS) of patients treated with TPF and PT, retrospectively (01/01/2015 to 31/12/2019), to establish a protocol for the treatment of locally advanced HNSCC at the Hospital de Clinics of the State University of Campinas. Methods: Demographic and clinical-pathological data, and outcomes to different regimens were obtained from the medical records by the researcher responsible for the study. Differences between groups were assessed using Fisher's exact probability or chi-square test, multiple logistic regression, Kaplan-Meier/log rank, and univariate and multivariate Cox analyses. Results: There were no clinical and pathological differences between 55 patients treated with TPF (n=32) and PT (n=23). We observed a higher response rate (60.9% versus 16.1%, p=0.009) and a higher SLE (66,6% versus 49%, p=0,02) in patients treated with the TP regimen than in those treated with the TPF. Twenty-five of 32 (78%) patients on the TPF regimen and 12 of 23 (52%) patients in the TP group received QT cycles with an interval equal to or greater than 28 days (p=0.042). No differences were observed in the number of QT cycles and toxicities to the regimens.
