Resumo: Maternal mortality represents a serious global public health issue. It is defined as the death of a woman during pregnancy, childbirth, or within 42 days after the end of pregnancy. Reducing global maternal mortality to less than 70 deaths per 100,000 live births by 2030 is one of the Sustainable Development Goals (SDGs). However, the COVID-19 pandemic had a significant negative impact, increasing maternal mortality rates in many countries, including Brazil, where COVID-19 was responsible for a substantial proportion of deaths among pregnant and postpartum women. Maternal mortality reflects socioeconomic inequalities and unequal access to maternal healthcare. In this context, the World Health Organization developed the term "Near Miss Materno," defined as a case in which a woman nearly died but survived a complication during pregnancy, childbirth, or the postpartum period. The aim of this study was to verify the characteristics of women reported by the Near Miss Materno monitoring system in the State of Paraná, identifying which characteristics increase the likelihood of death. This is a quantitative cross-sectional study conducted with data from the Near Miss Materno notification and monitoring system of the State Health Department and notifications from the Mortality Information System, covering the period from January to December 2021. The sample consisted of 938 pregnant women reported in the NMM, which were compared with the Mortality Information System. It was found that 7.78% had death as an outcome due to the worsening of NMM complications. Among the women who progressed to death, 61.9% were white. The average age was 28.67 (SD 6.31) years. Regarding health conditions, 38.1% had no pre-existing clinical conditions. The average gestational week at the time of notification was 33.06 (SD 6.08), and 42.9% of the women were classified as high risk gestational. In the analysis of clinical criteria, diagnoses of septic shock and severe acute respiratory syndrome (SARS) or COVID-19 had 4.04 (CI 1.60 to 10.24; p-value: 0.17) and 10.07 (CI 4.96 to 20.47; p-value: <0.001) times higher likelihood of progressing to death, compared to other variables. When comparing the group of women who survived the NMM with those who died, there were statistically significant differences in the number of children, diagnosis of hemorrhagic diseases, hypertensive diseases, SARS, or COVID-19. In conclusion, the main causes of maternal death were associated with direct and indirect obstetric complications that were not prevented, controlled, or avoided. The worsening of NMM may have occurred due to interactions with COVID-19 or due to complications during pregnancy, childbirth, or the postpartum period, which may have been exacerbated by a lack of hospital beds, reduced ICU stay duration, or discharges to wards of patients who were still not fully free of risk, due to the COVID-19 pandemic, compromising care for pregnant and postpartum women. Regarding the investigation of maternal deaths in Paraná, this study highlights the need for improvements in maternal healthcare services and greater accuracy in the information recorded in the state’s notification system.

É definida como o óbito de uma mulher durante a gestação, parto ou até 42 dias após o término da gravidez. Reduzir a mortalidade materna global para menos de 70 óbitos por 100.000 nascidos vivos até 2030 é um dos Objetivos de Desenvolvimento Sustentável (ODS). No entanto, a pandemia do Covid-19 teve um impacto negativo significativo, aumentando as taxas de mortalidade materna em muitos países, incluindo o Brasil, onde a Covid-19 foi responsável por uma parcela substancial de mortes em gestantes e puérperas. A mortalidade materna reflete desigualdades socioeconômicas e acesso desigual aos cuidados de saúde materna. Diante disso, a Organização Mundial da Saúde desenvolveu o termo Near Miss Materno, que é definido como um caso em que uma mulher quase morreu, mas sobreviveu a uma complicação durante a gravidez, parto ou puerpério. Diante disso o objetivo do estudo foi verificar as características das mulheres notificadas pelo sistema de monitoramento do Near Miss Materno no Estado do Paraná, identificando quais características aumentam a chance do óbito. Trata-se de uma pesquisa quantitativa de caráter transversal realizada com dados do Sistema de notificação e monitoramento do Near Miss Materno da Secretaria de Estado da Saúde e de notificações do Sistema de Informação Sobre Mortalidade, no período de janeiro a dezembro de 2021. A amostra foi composta por 938 gestantes notificadas no NMM, que foram confrontadas com Sistema de Informação Sobre Mortalidade e verificou se que 7,78% tiveram óbito como desfecho, devido ao agravamento das complicações do NMM. Das mulheres que evoluíram para o óbito 61,9% eram brancas. A média da idade era de 28,67 (DP 6,31) anos. Com relação a condição de saúde, 38,1% não possuíam condições clínicas preexistentes. A semana gestacional no momento da notificação era de 33,06 em média (DP 6,08) e 42,9% das mulheres foram classificadas como alto risco gestacional. Na análise dos critérios clínicos, os diagnósticos de choque séptico e SRAG ou Covid-19 tiveram 4,04 (IC 1,60 a 10,24. Valor de p: 0,17) e 10,07 (IC 4,96 a 20,47. Valor de p: <0,001) vezes mais chance de evoluir para o óbito, quando comparada com as outras variáveis. Ao comparar o grupo de mulheres que sobreviveram ao NMM com o grupo de mulheres que morreram, houve diferença estatisticamente significativa, para número de filhos, diagnóstico de doenças hemorrágicas, doenças hipertensivas, SRAG ou Covid-19. Conclui-se que as principais causas de Morte Materna foram associadas a complicações obstétricas diretas e indiretas, que não foram prevenidas, controladas ou evitadas. Os agravamentos do NMM podem ter ocorrido devido a interação com a Covid-19 ou decorrente de complicações na gravidez, parto ou puerpério, que podem ter sido agravadas, pela falta de leitos, redução do período de internamento na UTI, ou altas para enfermarias de pacientes ainda não totalmente livres de risco, devido a pandemia de Covid-19, prejudicando o atendimento às gestantes e puérperas. The worsening of NMM may have occurred due to interactions with COVID-19 or due to complications during pregnancy, childbirth, or the postpartum period, which may have been exacerbated by a lack of hospital beds, reduced ICU stay duration, or discharges to wards of patients who were still not fully free of risk, due to the COVID-19 pandemic, compromising care for pregnant and postpartum women. Regarding the investigation of maternal deaths in Paraná, this study highlights the need for improvements in maternal healthcare services and greater accuracy in the information recorded in the state’s notification system.
