Resumo: Background: Pneumococcal disease is one of the leading causes of death worldwide. Objective: To determine the epidemiology, serological and clinical response to PPV23 in children under 12 years of age who had recurrent airway infections, previously vaccinated with PMN10, followed at the immunodeficiency clinic of the Hospital Regional of Presidente Prudente-SP (HR). Materials: Medical records were analyzed; dosages of antibodies to PNM10 and to PPV23 were obtained before and after the application of the vaccine. Epidemiological aspects, comorbidities, immunological alterations were determined and the clinical improvement of patients after vaccination was evaluated. Results: There was an increase in antibody levels against different PPV23 serotypes after vaccination, as well as clinical improvement in 76% of patients, with a reduction in infectious processes, reduction in the use of antibiotics, and in emergency room visits. Conclusion: In the population studied, vaccination with PPV23 provided the improvement of patients and may represent savings for the SUS. The model can be applied to different services with poor resource settings, even without having a specialized outpatient clinic.

Objetivo: Determinar a epidemiologia, resposta sorológica e clínica à pnneumocócica não conjugada (PPV23) em crianças com idade inferior a 12 anos que apresentavam infecções de repetição de vias aéreas e que foram previamente vacinadas com PMN10, acompanhadas no ambulatório de imunodeficiências do Hospital Regional de Presidente Prudente - SP. Métodos: Através da análise de prontuários, foram obtidas as dosagens de anticorpos para PNM10 e para PPV23 antes e após a aplicação da vacina. Foram determinados aspectos epidemiológicos, comorbidades, alterações imunológicas e avaliada a melhora clínica dos pacientes após a vacinação. Resultados: Houve um aumento significativo nos níveis de anticorpos diante de diferentes sorotipos de pneumococcos presentes na PPV23 após a vacinação e melhora clínica em 76% dos pacientes, com redução dos processos infecciosos de vias aéreas e diminuição no uso de antibióticos e das visitas ao pronto socorro. Conclusão: Na população estudada, a vacinação com PPV23 proporcionou melhora clínica e da qualidade de vida dos pacientes e dos seus familiares.
