Utilização de plasma rico em plaquetas e rosuvastatina no processo cicatricial de feridas de coelhos: estudo longitudinal

Resumo: | Tissue engineering, as well as regenerative medicine, are fields that have been growing in recent years, aiming to repair, regenerate or replace damaged tissues and organs. PRP has been extensively studied in the areas of plastic surgery and dermatology with a focus on scar remodeling. Numerous studies with are still being done in search of new findings related to this blood element. Evidence is proving to favor the use of statins in skin lesions. In addition to lowering cholesterol, statins improve endothelial function, increase the stability of atherosclerotic plaques, and inhibit the inflammatory process. In this sense, the present study analyzed the collagen fibers of rabbit wounds after PRP treatment associated or not to RSV. The hypothesis is that serial use of RSV associated with PRP may improve the amount of neoformed collagen fibers after experimentally induced injury. 8 male New Zealand rabbits treated with autologous PRP, rosuvastatin (RSV) and association (PRP + RSV) were used to prepare the four wounds on the back of each rabbit. The treated wound was treated with 0.9% sodium chloride solution and the others treated with PRP isolated and / or associated with RSV. The dressings were changed every 4 days with application of PRP and rosuvastatin until 16 days of experiment. Biopsies were made at 0,7,10,14 and 17 days for histopathological evaluation of the scar tissue. Regarding the histopathological evaluation, at the end of 17 days of treatment, there was re-epithelialization in 100% of the wounds that received PRP only, opposing with 50% only with rosuvastatin and 75% when there was an association of the biomaterial with the statin. The combination of rosuvastatin and PRP inhibited blood loss. The use of PRP alone provided 62.5% of neovascularization, against 50% in wounds that had association of treatments. With regard to the percentage of collagen fibers at 170 days, all wounds had a higher percentage of fibers when compared to the control (78.27 ± 4.69). Although statistically equal, it was observed that the wound that received only RSV had the lowest amount of collagen fibers (85.98 ± 3.51). Wounds that received PRP or PRP and RSV presented 90.07 ± 6.20 and 90.76 ± 3.51, respectively. It is concluded that the PRP has greater activity in the first seven days of the lesion and that the treatment with RSV, alone or associated with PRP did not potentiate the other phases of healing, nor did it increase the percentage of collagen fibers. |

O PRP tem sido bastante estudado nas áreas de cirurgia plástica e dermatologia com enfoque na remodelação de cicatrizes. Numerosos estudos com ainda estão sendo feitos em busca de novas descobertas relacionadas a esse elemento sanguíneo. Evidências estão se mostrando favoráveis ao uso de estatinas em lesões de pele, Além de reduzir o colesterol, as estatinas melhoram a função endotelial, aumentam a estabilidade das placas ateroscleróticas e inibem o processo inflamatório. Neste sentido, o presente estudo analisou as fibras colágenas de feridas de coelhos após tratamento PRP associado ou não à RSV. A hipótese baseia-se que o uso seriado da RSV associado ao PRP possa melhorar a quantidade das fibras colágenas neoformadas após lesão experimentalmente induzida. Foram utilizados 8 coelhos da raça Nova Zelândia, machos, adultos, tratados com PRP autólogo, rosuvastatina (RSV) e associação (PRP+RSV) Para confecção das quatro feridas no dorso de cada coelho utilizou-se um punch de 8mm. A ferida considerada controle foi tratada com solução de cloreto de sódio a 0,9%® e as demais tratadas com PRP isolado e/ou associado à RSV. Os curativos forma trocados a cada 4 dias com aplicação do PRP e rosuvastatina até completarem 16 dias de experimento. Aos 0,7,10,14 e 17 dias foram feitas biópsias para avaliação histopatológica do tecido cicatricial. Com relação à avaliação histopatológica, verificou-se que, ao final de 17 dias de tratamento, houve reepitelização em 100% das feridas que receberam somente PRP, contrapondo com 50% só com rosuvastatina e 75% quando houve associação do biomaterial com a estatina. A associação da rosuvastatina ao PRP inibiu a perda de sangue. O uso do PRP isolado proporcionou 62,5% de neovascularização, contra 50% nas feridas que tiveram associação dos tratamentos. Com relação porcentagem de fibras colágenas ao 170 dia, todas as feridas apresentaram maior porcentagem de fibras quando comparadas ao controle (78,27±4,69). Embora estatisticamente iguais, observou-se que a ferida que recebeu somente a RSV apresentou a menor quantidade de fibras colágenas(85,98±3,51). Já as feridas que receberam PRP ou PRP e RSV apresentaram, respectivamente 90,07±6,20 e 90,76±3,51.
