Resumo: | Over time, the issue of mental disorders has been marked by stigmatization and intolerance toward those considered different from established social norms. Since the Psychiatric Reform, significant progress has been made in the mental health care model, enabling people with severe and persistent mental disorders to access dignified treatment. In addition to the suffering caused to the individual, their family is affected and must deal with a set of challenges, as the family is one of the key actors in the relationship of care and support for individuals with mental disorders. This study sought to understand the caregiving relationships in the aging process among people with severe and persistent mental illness and their family members, as well as their social support network. This study was conducted through a descriptive, qualitative field study using semi-structured individual interviews and the network map instrument proposed by Sluzki (1997). The research sample consisted of 12 individuals: six with mental illness who were users of the Psychosocial Care Center (CAPS) and six family members, one from each user participating in the study. The research resulted in two articles. The first, titled "Care Logics in the Context of Psychiatric Reform: Care Relationships of People with Mental Disorders and Their Families," aimed to understand care relationships in the aging process among people with severe and persistent mental disorders and their families. It revealed the dynamism present in the care interactions that make up the daily lives of these individuals and their relationship with mental health treatment. It demonstrated that the care relationships between individuals with mental disorders and their families can be understood from the perspective of the difficulties arising from mental illness, but also from the emotional bond present in these relationships. The results also indicate the importance of CAPS and reveal insecurities related to the future and aging of these subjects. The second article, entitled "Cartography of Support: The Significant Social Network of Users and Families of a CAPS," sought to understand the supportive relationships of users with mental disorders and their families based on their significant social network maps. The results reveal that the composition of the social networks of users with mental disorders and their families is dynamic, changing according to the context they experience, and they may rely on greater or lesser family and social support. It is evident that living with intense and chronic mental illness produces strain on the social networks of users and families, weakening their relationships and social participation. The data point to family as the primary supportive relationship for users and also indicate the CAPS as a highly important space for their care. However, they also highlight familism and the need to strengthen support networks for effective mental health care. In conclusion, we reiterate the need for public mental health policies to include families in care actions, promote spaces for social reintegration, and build mechanisms to strengthen support networks for mental health service users and their families. |

A partir da Reforma Psiquiátrica muito se avançou no modelo de cuidado em saúde mental para que as pessoas com transtornos mentais graves e persistentes pudessem acessar tratamento digno. Além do sofrimento gerado ao próprio sujeito, seu núcleo familiar é afetado e precisa lidar com um conjunto de desafios, pois a família é um dos principais atores na relação de cuidado e amparo ao sujeito com transtorno mental. O presente estudo buscou compreender as relações de cuidado no processo de envelhecimento entre pessoas com transtorno mental grave e persistente e seus familiares, bem como sua rede social de apoio. Para o desenvolvimento do presente trabalho foi realizado um estudo de campo, do tipo descritivo e com abordagem qualitativa, com a aplicação de entrevista individual semiestruturada e o instrumento mapa de rede, proposto por Sluzki (1997). A amostra da pesquisa foi composta por 12 indivíduos, seis com transtorno mental usuários do Centro de Atenção Psicossocial (CAPS) e seis familiares, um de cada usuário participante da pesquisa. A pesquisa deu origem a dois artigos. O primeiro, intitulado ¿Lógicas do cuidado no contexto da Reforma Psiquiátrica: as relações de cuidado de pessoas com transtorno mental e seus familiares¿, que objetivou compreender as relações de cuidado no processo de envelhecimento entre pessoas com transtorno mental grave e persistente e seus familiares, revelou o dinamismo presente nas interações de cuidado que compõem o cotidiano destes sujeitos e sua relação com o tratamento em saúde mental, evidenciando que o circuito de relações de cuidado dos sujeitos que possuem um transtorno mental e seus familiares pode ser compreendido a partir das dificuldades em decorrência do adoecimento psíquico, mas também pelo laço afetivo presente nestas relações. Os resultados também indicam a importância do CAPS e revelam inseguranças relacionadas ao futuro e envelhecimento destes sujeitos. O segundo artigo, chamado ¿Cartografia do amparo: a rede social significativa de usuários e familiares de um CAPS¿, buscou conhecer as relações apoiadoras de usuários com transtorno mental e seus familiares com base em seus mapas de rede social significativa. Os resultados revelam que a composição da rede social dos usuários com transtorno mental e seus familiares é dinâmica, modificando-se de acordo com o contexto vivenciado, podendo contar com maior ou menor suporte familiar e social. Evidencia-se que o convívio com o adoecimento psíquico intenso e crônico produz desgastes na rede social dos usuários e familiares, fragilizando suas relações e participação social. Os dados apontam a família como principal relação apoiadora para os usuários e também indicam o CAPS como um espaço de grande importância para seu cuidado. Porém, também evidenciam o familismo e a necessidade de fortalecer as redes de apoio para efetivação do cuidado em saúde mental.
