Resumo: Since the discovery of the JAK2V617F mutation in 2005, there have been questions about the role of the clonal allele burden, and its correlation to the clinic and prognostic features in the myeloproliferative neoplasm (MPN), especially Polycythemia Vera (PV). Results from the literature are, however, still heterogeneous, and inconclusive. Thus, we conducted a systematic review and meta-analysis from the articles published from 2005 to 2020, in PubMed and Scopus database, in general, 649 articles wore analyzed by two authors independently, after data wore crossed and 15 articles wore selected to answer our PICO and reach our methodological criteria and quality assessment. Overall, 2313 patients wore analyzed and 1529 tested and wore positive for JAK2V617F mutation. For the meta-analysis, three reviewers participated in data collection and statistical evaluation. Data on mutation prevalence, mean allelic burden, age, gender, presence of splenomegaly, leukocytosis, thrombosis, and progression to post-PV myelofibrosis and acute myeloid leukemia (AML) were evaluated. Prevalence data were confirmed in our statistics as well as the prevalence of mutations in homozygosis and heterozygosis. However, due to the great heterogeneity and lack of standardization of the data of each study, we were unable to accurately respond to our PICO, but the systematic review data confirm the association of increased allelic burden with the presence of leukocytosis, splenomegaly, and the evolution to post PV myelofibrosis.

Resultados da literatura permanecem heterogêneos e inconclusivos. Conduzimos então uma revisão sistemática da literatura e metanálise dos artigos publicados de 2005 a 2020, nas bases de dados do PubMed e Scopus, em geral 649 artigos foram analisados por dois autores de maneira independente e depois os dados foram cruzados. Destes artigos, 15 foram selecionados para responder nossa PICO e por atingirem nossos critérios de inclusão e avaliação de qualidade. Foram incluídos na análise 2313 pacientes dos quais 1529 foram testados e apresentavam a mutação JAK2V617F. Para metanálise três revisores participaram da coleta dos dados e avaliação estatística. Foram avaliados dados de prevalência da mutação, média de carga alélica, idade, sexo, presença de esplenomegalia, leucocitose, trombose e evolução para mielofibrose pós-PV e Leucemia mieloide aguda (LMA). Dados de prevalência foram confirmados em nossas estatísticas bem como a prevalência de mutações em homozigose e heterozigose.
