Mensuração linear de perda óssea periimplantar com e sem sobreposição de imagens radiográficas : estudo clínico metodológico

Resumo: Purpose: To evaluate the agreement of linear measurements of peri-implant bone loss with and without superimposition of radiographic images, depending on the arch (maxilla x mandible) and implant site (intercalary x free end). Methodology: The study sample consisted of 71 radiographic images of 44 single implants of 6 mm length, located in the posterior region of 19 patients. Standardized radiographs was taken after implant loading (T0) and 12 months after 12 loading (T1). The images were superimposed on the GIMP software and the linear measurements were performed on the ImageJ software. The bone level measurements were calculated by the mesial and distal means. Bone loss was calculated by subtracting the T1 values from T0. Data were assessed using intraclass correlation coeficiente (ICC). Results: The overall agreement between the methods was CCI = 0.88; stratifying by implant site ranged from CCI = 0.90 for free end and CCI = 0.89 for intercalary. As for the arcade, the ICC values were 0.82 in the maxilla and 0.90 in the mandible. Conclusion: The agreement between the techniques was high, suggesting that both techniques can be used in periimplant bone level research. However, the overlay of radiographic images adds another step in the image analysis process and, consequently, more operating time, which must also be considered when choosing the measurement technique.

Metodologia: A amostra foi composta por 71 imagens radiográficas de 44 implantes unitários de 6mm de comprimento, instalados na região posterior de 19 pacientes. As radiografias periapicais padronizadas foram obtidas após a instalação da prótese (T0) e após 12 meses em função (T1). As imagens foram sobrepostas no software GIMP e as medidas lineares foram realizadas no software ImageJ. O nível ósseo foi obtido como a média mesial e distal. A perda óssea foi calculada subtraindo-se os valores em T1 de T0. Os dados foram analisados por coeficientes de correlação intraclasse (CCI). Resultados: A concordância geral entre os métodos foi de CCI = 0.88; estratificando por local do implante variou de CCI = 0.90 para extremo livre e CCI = 0.89 para intercalar. Quanto à arcada, os valores de CCI foram de 0.82 na maxila e 0.90 na mandíbula. Conclusão: A concordância entre as mensurações de perda óssea utilizando as duas técnicas foi alta, sugerindo que ambas as técnicas podem ser utilizadas em pesquisa de nível ósseo peri-implantar.
