The purpose of this study was to evaluate soft tissue healing after mandibular third molar extraction using sutureless technique. In this prospective, split-mouth study, 39 patients were evaluated. The patients were assessed for postoperative soft tissue healing, pain and swelling. The primary outcome was postoperative evaluation of soft tissue healing at 1 week, using the Healing Index by Landry, and at 2 months after the procedure using pre and postoperative periodontal probing at the distal of the lower second molar. In the evaluation of secondary outcomes, the variables assessed were pain and swelling. Pain was analysed using a visual and numerical analogue scale (VAS). Swelling was assessed by comparing pre and postoperative extraoral measurements at 1 week using a flexible tape. The Wilcoxon and McNemar tests were used to assess for diferences in soft tissue healing, pain and swelling. The evaluation of the Healing index for soft tissue by Landry, showed a significant difference (p<0.01), suggesting that the use of sutureless technique leads to better soft tissue healing at one week after lower third molar surgery. The parameter for evaluation of periodontal tissue at the distal of lower second molar showed no statistical difference (p=0.5). The values obtained through the VAS for pain evaluation at days 1 (p=0.055), 3 (p=0.12) and 7 (p=0.058), showed no statistical difference. For the parameter swelling (p=0.3), there was no statistical difference between the groups. This study demonstrated that there is a positive correlation between the use of the sutureless technique and improvement in soft tissue healing after lower third molar surgery, and there is no statistical difference in periodontal probing depth at the distal of the lower second molar at 2 months. There is no statistical difference between the two techniques, in terms of reducing the amount of postoperative pain and swelling, during the first week after lower third molar surgery.
