Resumo: Elderly patients with chronic kidney disease undergoing hemodialysis are at greater risk of malnutrition due to dietary restrictions, intradialytic amino acid loss, anorexia and infectious intercurrences, resulting in worsening of health-related quality of life. The objective of this study was to evaluate the effect of treatment time and nutritional indicators on the quality of life of elderly patients with chronic kidney disease on hemodialysis. This was a descriptive, analytical, quantitative and cross-sectional study. The study sample consisted of 122 elderly people of both sexes, with chronic kidney disease and on hemodialysis for at least three months in two units of a renal clinic in the city of Santa Maria, RS, from March to July, 2018. Patients with acute renal failure, who had difficulty understanding the interview and who used enteral feeding, were excluded from the study. The data collection was performed through an interview and consultation of the patient's chart. The variables analyzed were socio- demographic data (age, sex, skin color, marital status, schooling and income), clinical history of renal disease (baseline disease, maintenance time on hemodialysis, associated comorbidities, nutritional indicators (anthropometric data, food, and biochemical tests) and quality of life. The Kolmogorov-Smirnov test, Student's t test, Mann-Whitney U test, Chi-square test, Pearson correlation test and Spermann correlation test were used for statistical analysis. The study was approved by the Research Ethics Committee of the Federal University of Santa Maria under No. 2,545,211, and the majority of the results were male (59.00 %), with a mean age of 70.31 ± 6.92 years, white (71.30%), companion (59.80%), schooling less than 5 years of study (68.00%), , income greater than 2 minimum wages ( 52.50%) and attended by the Unified Health System - SUS (72.10%). There was a significant difference in dietary intake in the elderly group with the highest hemodialysis time (≥3 years): g / day (p = 0.024), total calories (p = 0.034), carbohydrates , phosphorus (p = 0.029) and potassium (p = 0.016); biochemical tests: ferritin (p = 0.001), Kt / v (p = 0.010); there being no significance for anthropometric data and health-related quality of life among the elderly groups. Regarding the association between nutritional indicators and health- related quality of life, there was a correlation between the anthropometric variables and the categories: Team Stimulation / Dialysis, Social Support, Pain, Social Function, Energy / Fatigue and List of Problems / Symptoms; Physical function, Pain, General health, Welfare-emotional, Social function, Energy / fatigue, List of problems / symptoms, Renal disease overload, Sexual function and Team / dialysis stimulation; food consumption presented a correlation of macronutrients with Physical functioning, Physical function, Pain, General health, Well-emotional, List of problems / symptoms, Cognitive function and Sexual function and Micronutrients with Physical functioning, Physical function, Pain, General health, Emotional well-being, Emotional function, Social function, Energy / fatigue, List of problems / symptoms, Cognitive function, Quality of social interaction and Sexual function. It was concluded that the elderly with a longer time of hemodialysis treatment presented higher risk for protein-energy malnutrition, high phosphorus, low potassium intake and elevated ferritin, a behavior not observed in quality of life. Still, nutritional indicators directly influenced the health-related quality of life categories of the elderly.

Still, nutritional indicators directly influenced the health-related quality of life categories of the elderly.Coordenação de Aperfeiçoamento de Pessoal de Nível Superior - CAPESOs idosos com doença renal crônica em hemodiálise estão sujeitos a maiores riscos de desnutrição, devido a restrições dietéticas, perda de aminoácidos no período intradialítico, anorexia e intercorrências infecciosas, resultando na piora da qualidade de vida relacionada à saúde. O estudo teve como objetivo avaliar o efeito do tempo de tratamento e indicadores nutricionais na qualidade de vida de idosos com doença renal crônica em hemodiálise. Tratou-se de um estudo descritivo, analítico, quantitativo e de abordagem transversal. A amostra do estudo foi composta por 122 idosos de ambos os sexos, com doença renal crônica e em hemodiálise há pelo menos três meses em duas unidades de uma clínica renal na cidade de Santa Maria, RS, no período de março a julho de 2018. Foram excluídos da pesquisa os pacientes com insuficiência renal aguda, com dificuldade de compreensão na entrevista e que utilizavam alimentação enteral. A coleta dos dados foi realizada por meio de entrevista e consulta do prontuário do paciente. As variáveis analisadas foram os dados sociodemográficos (idade, sexo, cor da pele, estado conjugal, escolaridade e renda), histórico clínico da doença renal (doença de base, tempo em manutenção em hemodiálise, comorbidades associadas, indicadores nutricionais (dados antropométricos, consumo alimentar e exames bioquímicos) e qualidade de vida. Para análise estatística dos resultados foram utilizados os testes Kolmogorov-Smirnov, Teste t de Student, Teste U de Mann-Whitney, Teste de Qui-quadrado, Teste de correlação Pearson e Teste de correlação Spermann. O nível de significância adotado foi de 0,05. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da Universidade Federal de Santa Maria sob o nº 2.545.211. Quanto aos resultados obtidos, a maioria era do sexo masculino (59,00%), com média de idade de 70,31±,6,92 anos, de cor branca (71,30%), com companheiro (59,80%), escolaridade inferior a 5 anos de estudo (68,00%), renda maior do que 2 salários mínimos (52,50%) e atendidos pelo Sistema Único de Saúde – SUS (72,10%). Ainda foi demonstrado que no grupo de idosos com maior tempo de hemodiálise (≥3 anos) houve diferença significativa no consumo alimentar sendo: proteínas g/dia (p=0,024), calorias totais (p=0,034), carboidratos (p=0,046), fósforo (p=0,029) e potássio (p=0,016); exames bioquímicos: ferritina (p=0,001), Kt/v (p=0,010); não havendo significância para os dados antropométricos e qualidade vida relacionada à saúde entre os grupos de idosos. Quanto a associação entre os indicadores nutricionais com a qualidade de vida relacionada a saúde, houve correlação entre as variáveis antropométricas e as categorias Estímulo de equipe/diálise, Suporte social, Dor, Função social, Energia/fadiga e Lista de problemas/sintomas; os exames bioquímicos com Funcionamento físico, Função física, Dor, Saúde geral, Bem estar-emocional, Função social, Energia/fadiga, Lista de problemas/sintomas, Sobrecargas da doença renal, Função sexual e Estímulo equipe/diálise; o consumo alimentar apresentou correlação dos macronutrientes com Funcionamento físico, Função física, Dor, Saúde geral, Bem estar-emocional, Lista de problemas/sintomas, Função cognitiva e Função sexual e os micronutrientes com Funcionamento físico, Função física, Dor, Saúde geral, Bem estar-emocional, Função emocional, Função social, Energia/fadiga, Lista de problemas/sintomas, Função cognitiva, Qualidade da interação social e Função sexual. Conclui-se que os idosos em maior tempo de tratamento hemodialítico apresentam maior risco para desnutrição energético-proteica, um consumo elevado de fósforo, baixo de potássio e elevação da ferritina, comportamento não observado na qualidade de vida.
