Resumo: The Obstructive Sleep Apnea Syndrome (OSAS) is high prevalent and has as consequences excessive daytime sleepiness, risk of labor and traffic accidents, cognitive impairment, mood alterations, metabolic and cardiovascular diseases that compromise the health, the quality of life and surveillance of the patients. Still, the syndrome is not always adequately diagnosed, due especially to physicians’ limited acquaintance, patients’ misperception of signs and symptoms and to the complexity of diagnostic methods. In this dissertation it will be presented and discussed the main publications of my line of research concerning limitations on OSAS diagnostic approaches. The polysomnography (PSG) is considered the gold standard method for evaluating the syndrome; nevertheless, the reproductibility of measured parameters during the exam had not been completed evaluated. On my doctorate thesis, I attested that there is an individual variability of the apnea and hypopnea index (AHI), the metric measure used in the PSG for OSAS’s diagnose and treatment follow-up. Thereafter we sought to develop a systematic evaluation model of the upper airway that could simply and practically contribute to a diagnostic approach. Such model demonstrated a significant relationship with the presence and severity of OSAS in a sample of patients presenting the syndrome. Later on, in other two studies, one comparing patients and controls, and other comparing obese with and without OSAS, the model was able to differentiate groups and to demonstrate relationship to OSAS independently of obesity. In addition to this line of research, we demonstrated the accuracy of portable devices for cardio-respiratory ambulatory register, in diagnosing patients with high clinical probability for OSAS. With these results we prove the validity of simplified clinical and register methods for OSAS.

With these results we prove the validity of simplified clinical and register methods for OSAS.A Síndrome da Apneia Obstrutiva do Sono (SAOS) é prevalente e tem como consequências a sonolência excessiva, o risco de acidentes de trabalho e de trânsito, os déficits cognitivos, as alterações de humor, as doenças metabólicas e as cardiovasculares que comprometem a saúde, a qualidade de vida e a taxa de sobrevida dos pacientes. Nem sempre a SAOS é diagnosticada adequadamente, devido principalmente ao desconhecimento desta doença por parte dos médicos, à má percepção dos sinais e dos sintomas pelos pacientes e pela complexidade dos métodos diagnósticos. Nesta dissertação apresentam-se e discutem-se as principais publicações da minha linha de pesquisa sobre as limitações nas abordagens diagnósticas da SAOS. A polissonografia é considerada o método diagnóstico ideal para avaliar a existência desta síndrome, mas a reprodutibilidade dos parâmetros medidos durante esse exame ainda não tinham sido totalmente avaliada. Na minha Tese de Doutorado, comprovei que existe uma variabilidade individual do índice de apneia e hipopneia (IAH), que é a medida métrica utilizada na polissonografia para o diagnóstico e o acompanhamento do tratamento da SAOS. A partir daí, procuramos desenvolver um modelo sistemático de avaliação da via aérea superior que, de forma mais simples e prática, pudesse contribuir para uma melhor abordagem diagnóstica. Tal modelo demonstrou uma relação significativa com a presença e gravidade da SAOS numa amostra de pacientes com a síndrome. Posteriormente em outros dois estudos, um comparando pacientes e controles, e outro comparando obesos com e sem apneia, o modelo foi capaz de diferenciar os grupos e se mostrar relacionado à SAOS independente da presença da obesidade. Posteriormente, complementando essa linha de pesquisa, demonstramos a precisão que os aparelhos portáteis de registro ambulatorial cardio-respiratório têm, para diagnosticar quais são os pacientes com alta probabilidade clínica de terem a SAOS.
