Profilaxia pós-exposição (PEP) ocupacional ao HIV e aos vírus das hepatites B e C em serviço de referência

Resumo: Occupational exposures to biological materials pose a significant health risk to professionals, especially regarding infection with HIV, hepatitis B virus (HBV), and hepatitis C virus (HCV). This study aimed to analyze adherence to post-exposure prophylaxis (PEP) and outpatient follow-up at a referral service in the interior of São Paulo State, Brazil. This was a retrospective observational study based on the analysis of 238 medical records of exposed workers treated between 2017 and 2022. Most of the exposed individuals were female (71.4%), predominantly nursing technicians (33.2%) and cleaning assistants (18.1%). Blood was the main fluid involved (74.4%), and sharp object injuries were frequent. Among the 154 professionals prescribed PEP, 92.2% adhered correctly to the treatment, and 93.3% completed follow-up. The high adherence rate indicates the effectiveness of initial actions; however, follow-up dropout and incomplete records reveal weaknesses in the monitoring system. It is concluded that, although the service shows good initial indicators, there is still a need to strengthen institutional policies, continuously train teams, and standardize protocols to ensure greater safety for professionals and a better response to occupational exposure events.

Este estudo teve como objetivo analisar a adesão à profilaxia pós-exposição (PEP) e ao acompanhamento ambulatorial em um serviço de referência do interior paulista. Trata-se de um estudo observacional retrospectivo, com base na análise de 238 prontuários de trabalhadores expostos atendidos entre 2017 e 2022. A maioria dos acidentados era do sexo feminino (71,4%), com predomínio de técnicas de enfermagem (33,2%) e auxiliares de limpeza (18,1%). O sangue foi o principal fluido envolvido (74,4%) e os acidentes com agulhas ocorreram com mais frequência. Entre os 154 profissionais com PEP prescrita, 92,2% aderiram corretamente ao tratamento e 93,3% concluíram o acompanhamento. A alta taxa de adesão indica efetividade das ações iniciais, porém a evasão de seguimento e a existência de prontuários incompletos revelaram fragilidades no sistema de acompanhamento.
