Prevalence of Molar Incisor Hypomineralization in children with cleft lip and palate

Resumo: Dental alterations are commonly diagnosed in children with cleft lip and palate and are linked to disorders arising from cleft-related genes, as the formation of dental germs coincides with the cleft formation period. Some alterations are often studied, but there is still limited evidence in the literature about Molar Incisor Hypomineralization (MIH) and its relationship with cleft lip and palate. Therefore, the aim of this study was to assess the prevalence of MIH in 6-12-year-old non-syndromic children with cleft lip and palate, and to compare the prevalence between 2000 and 2015 to determine if there was an increase or decrease in cases. The study was conducted in Bauru at the Pediatric Dentistry and Public Health Department of the Craniofacial Anomalies Rehabilitation Hospital (HRAC). The sample was population-based, including 478 children aged 6 to 12 with fully erupted first molars and permanent incisors. Considering the children\'s birth years, we obtained 350 children born in 2000, 22 in 2015, and 106 in the interval between those years. Data collection involved analyzing the patients\' photographic documentation available in the hospitals digital system. The examination and diagnosis were performed by two previously calibrated examiners, using the extended Ghanim et al. index for MIH. In addition to MIH, data on gender, age, defect severity, and cleft type (Spina classification, 1992) were collected. Statistical analysis was performed using multiple linear regression and chi-square tests, considering both teeth and individuals as sample units. Results showed an overall MIH prevalence of 11%, with specific prevalences of 12.6% in 2000 and 13.6% in 2015, without a statistically significant increase over time.

Algumas das alterações são frequentemente estudadas, porém, ainda não há muitas evidências na literatura sobre a Hipomineralização Molar Incisivo (HMI) e a sua relação com a fissura labiopalatina. Portanto, o objetivo desse trabalho foi avaliar a prevalência de Hipomineralização Molar Incisivo em crianças de 6 a 12 anos com fissura labiopalatina e não sindrômicos, bem como realizar um comparativo entre os anos de 2000 e 2015 para concluir se houve um aumento ou redução no número de casos. O estudo foi realizado na cidade de Bauru, no setor de Odontopediatria e Saúde Coletiva do Hospital de Reabilitação de Anomalias Craniofaciais (HRAC). A amostra foi de natureza populacional com um total de 478 crianças de 6 a 12 anos de idade que apresentaram primeiros molares e incisivos permanentes completamente irrompidos. Considerando o ano de nascimento das crianças, obtivemos 350 crianças nascidas nos anos de 2000, 22 em 2015 e 106 no intervalo entre esses anos. A coleta de dados foi feita por meio da análise das documentações fotográficas dos pacientes disponíveis no sistema digital do Hospital e o exame e diagnóstico foram realizados por dois examinadores previamente calibrados, utilizando o índice de Ghanim et al. ampliado para HMI. Além da HMI, foram coletadas informações sobre gênero, idade, grau de severidade do defeito e tipo de fissura, conforme a classificação de Spina (1992). A análise estatística foi realizada utilizando regressão linear múltipla e teste qui-quadrado, considerando tanto os dentes quanto os indivíduos como unidades amostrais.
